Objective:To introduce and assess the feasibility, safety and efficacy of transurethral bipolar plasmakinetic combined with green laser in treatment of large volume benign prostatic hyperplasia(BPH).Material and Method:Between June2009 and January 2010, a total of 32 patients (66~83, mean 72.3±4.6 years old; with other complications) with large volume (89-243ml, mean 122.6±32.1ml) benign prostate hyperplasia (BPH) from the Minimal Invasive Urology Center of Shandong Province Hospital were treated with the KTP-laser photoselective vaporization of the prostate(PVP) combined with transurethral plasmakinetic resection of the prostate (PKRP) or PVP only.32 patients were treated in-two groups, Group 1 (n=16):PVP only, and group 2:(n=16):PKRP combined with PVP. PKRP was first applied and then PVP performed on the patients. In cases where there was obvious angiectasis on the surface of prostate. PVP was first applied to stop bleeding before PKRP was performed on prostate and after that PVP was applied again.Results:All the operations were successful,and none of the patients needed blood transfusion or had complications. PVP+PKRP group had shorter operation time, less inpatient expenditure, longer catheters removed time and length of stay than PVP group (p <0.05). more patients needed bladder irrigation than who underwent PVP.No significant difference was found between 2 groups at Qmax, IPSS, QOL(p>0.05), while it was significantly improved than that pre-operation(p<0.05).Conclusion:PKRP combined with PVP,in possession of the advantages of both PKRP and PVP.It has less trauma, faster recovery, less complications than PKRP,and shorter operation time, less inpatient expenditure than PVP,it also can provide prostate tissue for the pathological examination.There is no significant difference at short-time postoperative curative effect between PKRP+PVP and PVP.It is a safe and effective treatment of BPH, particularly in the case of patients with large prostates.
