| Objective:Through raising animal model, we imitate to Survey and compare the effect of jia di hai yin solution and biomembrane named man-made cerebral dura mater on some changes, fibroplasias aroud spinal dura mater, and nerve root adhes. By way, it will be the determinate experiment base that guide clinical medication.Methods:1.60 famle Wistar rats, with the average weight of 230-270g, were randomly divided 4 groups:jia di hai yin solution group (A), man-made cerebral dura mater group (B), jia di hai yin solution+man-made cerebral dura mater group (C), Sodium Chloride control group (D).2. Firstly, the whole number experienced suprapubic cystostomy separately. Urodynamics and electromyogram were given to them in the next week. Two or three days later, the second operation was time. To cut open every layer in turns taking L3 as the center, spinal dura mater was windowed 0.3cm*0.1cm, ringing clamp 3 places, with spinal dura mater damaged and cerebrospinal fluid unleakage, A group spread-ed on jia di hai yin solution 0.5ml, B group covered man-made cerebral dura mater, C group overlaid man-made cerebral dura mater steeped jia di hai yin solution; D group applying Sodium Chloride 0.5ml; 5 rats were randomly selected from every group, undergoing Urodynamics and electromyogram at the first week, fourth week, and the eighth week, after the second operation. Then, to put them to death, cutting open every layer in turn, in accordance with Rydell grading standard, and viewing the adhesions condition of spinal cord and nerve root in spinal dura mater; taking out of the whole vertebral column sample piece(L2-L4), common Decalcification, dehydration, transparency and paraffin imbedding, serial section for 5um, HE staining, according to improving Nussbaum score of histology, finally viewing the status of conglutination in spinal dura mater through light microscope. Based on the score Tarlov standard,all rats hind limb ethology function is evaluated. Among all handled groups, and at the same time campared to control group, applied variance analysis, when P< 0.05, it is statistics meaning.Results:1. Observation on sample:in accordance with Rydell grading standard, after 1 weeks, all groups main in 0ï½žâ… grade, raritas and sporadic adhes are found exterior and interior cerebral dura mater, scar tissue is easy to be bluntly dissected from dura mater membrane vesicle,there is no significant deviation (P>0.05) among postop groups. But after 4 weeks, scar tissue and adhere covers generally exterior and interior cerebral dura mater, scar tissue is difficult to be bluntly dissected from dura mater membrane vesicle, with vesicle integrated, D group is main in I~Ⅱgrade, and A,B,C main in 0-â… grade; the significant deviation occurred:between A and D, B and D, C and D(P< 0.05); but A and B, A and C, B and C. Finally, scar tissue and adhere covers generally exterior and interior cerebral dura mater, scar tissue is difficult to be bluntly dissected from dura mater membrane vesicle, with vesicle not integrated, D group sored forⅡ~Ⅲgrade, but A,B and C groups adhere gently, there is significant deviation(P<0.05) between B and D, C and D;but A and D, B and C (P>0.05)2. Pathological section HE staining:according to improving Nussbaum score of histology, after 1 weeks, the adhes area of spinal cord, is less to 25%, Collagen sparseness, scar and graft rarely extend to side or rear, every group ranks as 0 grade, there is no significant deviation (P >0.05) among postop groups. but after 4 weeks, the adhes area grows approximately to 25%-50%, spinal cord and nerve root adhere inner cerebral dura mater, Collagen sparseness, scar sides to the place between vertebral plate defect and spinal cord, or partly insert into vertebral plate defect, without spinal cord compression, D group is main in I grade, but A,B and C groups adhere gently, the significant deviation occurred:between A and D, B and D, C and (P<0.05); but A and B, A and C, B and C. finally, the adhes area spreads approximately much more than 50%, Collagen sparseness outside of spinal cord;scar circumvolutios, or stretchs to one side nerve root, scar inserts into vertebral canal; and spinal compression, deformation, spinal cord and nerve root adhere inner cerebral dura mater, D group sored for II grade, butB and C groups adhere gently, there is significant deviation(P<0.05) between B and D, C and D, A and B, A and C, B and C.3. Functional evaluation on posterior limb ethology:all test rats run normally after the first operation, scored as 5; whereafter, the score decreased to 4 in the first week after the first operation, but there is no significant deviation (P>0.05)among groups; it continued cutting down.score as 3, each group occurs paralysis at different level, hind limb acting frequently and powerfully, without weight loading, the level of paralysis in A,B and C group is less than D, the significant deviation occurred:between A and D, B and D, C and D(P<0.05), but not between A and B, A and C, or B and C; finally, the D group rats paralyzed severely, tendency of the myosthenic descent running forward, and ulcer can be found in hind limb, only light activity, it changed to 2 grade, the level of paralysis in A,B and C group is less than D, there is significant deviation(P<0.05) between B and D, C and D, and A and B, A and C, B and C.4. Study on Urodynamics:after the first operation, to compare bladder capacity, pressure of detrusor and compliance, there is no significant deviation (P>0.05) among preop groups; groups with the second postop ones, bladder capacity and pressure of detrusor increased with compliance lowered, the deviation isn't obvious (P>0.05) in the first week after the second operation; the significant deviation occurred:between A and D, B and D, C and D(P<0.05), but not between A and B, A and C, or B and C; finally, there is significant deviation(P<0.05) between B and D, C and D, the same as A and B, A and C, B and C.5. Study on somatosensory evoked potential (SEP):comparing the latency and amplitude of N17and P40, there is no significant deviation (P>0.05) among groups after the first operation; in the first week after the second operation, the difference between preop groups and postop ones is prominent (P<0.05), but there is no significant deviation (P>0.05) among postop groups. A, B and C postop groups, compared with preop ones, have significant deviation (P<0.05) in the fourth and eighth week after the second operation, the same as between A and D, but B with D and C with D; A and B, A and C, B and C. finally, there is significant deviation(P <0.05) between B and D, C and D, the same as A and B, A and C, B and C.Conclusion:It is initial feasibility by jia di hai yin solution, but in advanced stage, the curative effect need to be further studied. Man-made cerebral dura mater could effectually prevent the cicatrix adhes around spinal durra mater postop of myelodysplasia; The function, Man-made cerebral dura mate associated jia di hai yin solution, is distinguished。Therefore, it is possible to guide clinical medication. |
