With the wider use of mechanical ventilation, The common complication of mechanical ventilation-associated pneumonia (VAP) more and more attention. According to reports, VAP is a nosocomial pneumonia, a special type, there is a high incidence and mortality. VAP incidence rate was 9% to 70% and the mortality was 50% to 69%, thus reducing the incidence of VAP, mortality, have some clinical significance.In this paper, artificial nose and closed-type suction pipe to reduce the ICU ventilator-associated pneumonia and to prevent VAP occurrence are further studied and discussed. Object of study options from July 2008 to September 2009 patients with ICU stay, and ventilation≥48 hours, age≥l6 years old, were randomly divided into four groups A, B, C, D four groups. A group , 30 patients with artificial nose and the joint sealed and suction tube group; B group, 30 cases of artificial nasal group (open-ended suction tubes) ; C group, 30 cases of closed-type suction tube group; D group of 30 cases for the traditional treatment group that is not to install artificial nose, non-use of closed suction tube, used as control group. Reference to the U.S. Centers for Disease Control (CDC) on the standard of hospital-acquired pneumonia, combined with acute and chronic physical health score and pulmonary infection score (CPIS), CPIS≥6 is divided into VAP standards.OUTCOME MEASURES: 1 incidence of Ventilator-associated pneumonia 2 mechanical ventilation time 3 Hospitalization time in ICU 4 Deep tracheal sputum bacterial culture 5 adverse events caused by the suction: cardiac arrhythmia, hypoxemia, the incidence of pulmonary wilt.Adopt a unified approach to data collection, research purposes according to the unified design specification tabulation, including the airway pressure of each group of patients, VAP incidence, mortality, total bacteria counts, mechanical ventilation time and ICU length of stay, suction lead to adverse events, etc.Unicode all the information, using EXCEL database, data entry, using SAS 9.0 statistical software for data statistical analysis.Results: Comparison of clinical data of the four groups in general (see Table 1, Table 2). A, B, C, D four groups by age, sex, acute and chronic physical health score (APACHII), antibiotics, and triple triple over the use of antacids did not differ significantly (p> 0.05). Artificial Nose Joint Closed suction tube group VAP incidence rate is lower than that of artificial nose group, sealed and suction tube group, open-ended suction tube group, the incidence was significantly higher than the other three. ICU length of stay shortened, VAP mortality no significant difference.Therefore, the joint of artificial nose Closed suction pipe in combination can reduce the incidence of VAP, extend the time of onset, shorten ICU hospitalization time, reducing hospital stay. It is worthy of clinical use.
