Preliminary Study For 64-slice Spiral CT Perfusion Imaging Of Hepatic Hemodynamic Changes With Obstructive Jaundice

Objective To investigate the value of CT perfusion imaging in study of hepatic hemodynamic changes with obstructive jaundice.Methods Siemens 64-slice spiral CT perfusion scans of liver was performed in 18 volunteers without hepatic disorder and 21 patients with obstructive jaundice.Arterial liver perfusion(ALP),portal venous perfusion(PVP), hepatic perfusion index(HPI),blood flow(BF),blood volume(BV) and permeability(P) were measured and statistically analyzed,using Siemens Body perfusion software package.Results The data of ALP,PVP,HPI,BF,BV and P in the control group were (27.32±5.44) ml·100ml-1·min-1,(109.09±9.27) ml·100ml-1·min-1, (20.46±2.69)%,(48.84±4.82) ml·100ml-1·min-1,(135.73±3.24) ml/L, (70.12±6.14) 0.5ml·100ml-1·min-1,respectively.The data of ALP,PVP, HPI,BF,BV and P of liver in the obstructive jaundice were(32.55±6.75) ml·100ml-1·min-1,(104.82±8.76) ml·100ml-1·min-1,(23.52±3.44)%,(49.29±5.67) ml·100ml-1·min-1,(133.35±3.80) ml/L,(62.54±7.24) 0.5 ml·100ml-1·min-1,respectively.Statistical analysis showed:the values of ALP,HPI in the obstructive jaundice were higher than those of the control group,and the difference was significant(P<0.05),with the progress of jaundice,they having trend to increase.The difference of ALP and HPI between second-stage and control group were statistically significant(P<0.05),and the difference of the value of ALP between second-stage and first-stage were statistically significant(P<0.05);the value of BV and P decreased compared to the control group,and the difference was significant(P<0.05),and with the different jaundice degrees,it decreased gradually.the differences of BV and P between second-stage and control group were statistically significant(P<0.05), and the difference of the value of P between first-stage and control group were also statistically significant(P<0.05);However,the means of PVP and BF between groups were not statistically significant(P>0.05).Conclusion Hepatic perfusion imaging with 64-slice spiral CT can reflect changes of hepatic blood perfusion of obstructive jaundice,and be valuable for early diagnosis and treatment of clinical.
