Part 1 Study in hair removal on scrotal septum skin of ratObjectives: To investigate the histological and physiological change of scrotal septal skin and testis of rat after 650nm IPL and 1064nm long-pulsed laser irradiation. Materials and methods: After hair removal by shaver, the left side of scrotal septal skin of BN rat was irradiated with 650nm IPL laser while the right side was irradiated with 650nm IPL and 1064nm long-pulsed laser.21 BN rats were divided into 7 groups. Group 1 to 6 were treated by laser only once. Scrotal septal skin and testis were examined on the same day, 1 day,3 days,7 days, 14 days,28 days after irradiation. Group 7,the control group, didn't receive any irradiation. Scrotal septal skin and testis were examined on the same day, 14 days,28 days after hair removal.Results: Comparing with control group, the hairs of regrowth became thinner and lesser after one irradiation. Immediately after laser irradiation, light microscope revealed coagulation necrosis and nuclear structure of partial bulbs of hair, some of which with deformed contour. Several days later, light microscope revealed decreased section proportion of bulbs and stems of hair in hair follicles, deficit of hair shafts in more follicles, remained only hair root sheaths and an empty cavity. Compared to normal control with no obvious difference, normal appearance and clear structure of testis achieved.Conclusion: Using a proper fluence, satisfactory hair removal were achieved in all BN rats. No damage was observed in BN rat testis after 650nm IPL and 1064nm long-pulsed laser irradiation. Part2 The clinical use of scrotal septum flap after hair removal to the repair hypospadiasObjectives: The purpose of our study is to explore the use of scrotal septum flap in the repair of hypospadias. The scrotal septum skin was depilated by intense pulsed light and long-pulsed laser to avoid the complication of hair growth in the reconstructed urethra.Materials and methods: Between June 2007 and January 2008, 3 male patients with hypospadias were depilated by intense pulsed light and long-pulsed laser in the scrotal septum skin, whose ages ranged from 18 to 19 years .The operation was divided into two stages: the oral mucosa graft was taken to prefabricate distal urethra in the first stage ; the second stage was to reconstruct defect urethra with scrotal septum flap after hair removal. The time interval between two stages was 6-15 months. During the interval, the patients were treated with hair removal by using 650nm intense pulsed light (IPL) and 1064nm long-pulsed laser.Results: Hair reduction in scrotal septum skin was observed in all patients after three treatments. New hair growth after treatments was less and delayed. All patients were followed up 4 to 7 months after the second operation. No complications like urethral fistula and hair growth in the reconstructed urethra were observed. After three treatments, light microscope revealed the follicle atrophy remained only hair root sheaths and anempty cavity.Conclusions: The hair removal using IPL and long-pulsed laser in scrotal septum skin is an ideal method because of its credible effect. This method can avoid the complication of hair growth in the reconstructed urethra.
