Backgroud:Liver transplantation nowadays is a well accepted and effective treatment option for end-stage liver disease.How to decrease venous pressure of abdominal viscera during anhepatic phase,maintain intraoperative hemodynamic stability,reduce the intraoperative use of vasoactive drugs,speed up the recovery of intestinal function and so on,will be the an important research direction for the field of liver transplantation.Objective:To investigate the effect and mechanism of somatostatin in protecting intestine during perioperative period of liver transplantation in rats.Materials and Methods:Twenty Lewis rats were randomly divided into two groups:control group and somatostation group.All of the rats underwent two-cuff technique for orthotopic liver transplantation.The rats of somatostation group were injected with somatostatin (20ug/kg,via penile dorsal vein)at three time points:5minutes before the blocking of portal vein,at the time of the blocking,5 minutes,10min,15min after the blocking. Then changes of portal pressure were monitored at the same points.Blood was taken from suprahepatic vena cava(SH-VC) for malondialdehyde(MDA) detection at different time points after transplantation(5min,20min,30min,1h,2h,6h). Continuously,activity of MPO in blood from SH-VC,while activity of MPO, Na+/K+-ATP enzyme and the level of MDA of intestinal and hepatic tissue were estimated.A section of the middle segment of the liver was sampled and fixed in 10% formaldehyde for pathological examination.Results:The portal pressure decreased in the rats treated with somatostation.MDA concentration in SH-VC elevated obviously with time and peaked at 1h after opening of portal vein.Activity of MPO in blood decreased extremely after somatostation treatment(p<0.05).Activity of MPO and the level of MDA of intestinal and hepatic tissue in somatostation group were obviously lower than control group,while Na+/K+-ATP enzyme increased conversely.(intestinal tissue:p<0.01;hepatic tissue p<0.05)Conclusion:Somatostation can reduce hepatic/intestinal congestion and ischemia-reperfusion injury during the perioperative period of liver transplantation in rats,thereby protect intestinal absorption.As a gastrointestinal peptides,it can reduce pressure on circulation during the perioperative period of liver transplantation,prevent neutrophil infiltration and inhibit the release of oxygen free radicals,consequently avoid the lipid peroxidation.
