Objective:To investigate the correlation among hepatitis B virus markers (HBVM) in gastric juice,serum and gastric mucosa in chronic patients with hepatitis B virus infection,the influence of HBV infection on gastric mucosa in chronic hepatitis B patients,and the possibility of infection by alimentary canal.Methods:60 patients with chronic HBV infection diagnosed by serum HBV DNA examination were enrolled in this study.All patients were undertaken gastroscopy,liver function test,and HBVM examination.HBVM in Serum and gastric juice were determined with ELISA while HBsAg and HBcAg in gastric mucosa were detected by immunohistochemistry. Results:1.The positive rates of HBsAg and HBeAg were,respectively,80%and 25%in the gastric juice of 60 cases of chronic patients with HBV infection.There is no significant difference between the HBV levels in gastric juice and serum(p>0.05).HBV was found in gastric juice in some patients without HBsAg,HBeAg and anti-HBc in serum.2.In 60 patients,the positive rate of HBVM in the gastric mucosa in patients with HBV-associated cirrhosis was highest and the positive rate of HBVM in chronic hepatitis B was higher than that in chronic asymptomatic HBV carrier.3.HBcAg and(or) HBsAg were found in gastric mucosa of 29 patients.2 cases had only HBsAg,10 cases had only HBcAg,and 17 cases had both HBsAg and HBcAg in the gastric mucosa.HBVM in serum were not related to those in gastric mucosa,but HBsAg in gastric juice were related to those in gastric mucosa.4.The damage of gastric mucosa was found in 56 of 60 patients.In 20 of 29 cases with HBcAg and(or) HBsAg in the gastric mucosa,there were moderate and severe gastric damage,whereas 22 of other 31 cases without HBcAg and(or) HBsAg in the gastric mucosa showed mild and moderate damage.The damage of the gastric mucosa in the patients with positive findings of HBVM in the gastric mucosa was serious.Conclusions:1.HBV may grow and copy in gastric mucosa,maybe it is an important factor of gastric mucosa lesions in chronic patients with HBV infection.The damage of gastric mucosa in the patients with HBV infection is correlation with the infection of HBV in the gastric mucosa.2.HBVM could be detected in gastric juice,so it is possible that HBV is infected via gastric juice.It is necessary to prevent infection from blood as well as alimentary canal.
