Since entering the 21st century, with the deepening of reform and opening up, China's lack of co-ordination of urban and rural development have become increasingly conspicuous. Among them, in the medical security, the vast majority of the rural population has no health insurance. Therefore, "poor for illness, illness returning to poor" is a more serious phenomenon in rural areas. To resolve the issue of rural health care has again been put on the agenda.The CPC Central Committee and State Council in 2002 at " further strengthen health work in rural areas " made it clear that by 2010, our country will set up the basic rural health service system and rural cooperative health care system throughout the country to adapt to the socialist market economic structure and rural economic development level. The new rural medical insurance is a major policy decision and initiative of the CPC Central Committee. Which aimed at achieving the goal of comprehensively building a well-off society, the coordination of urban and rural and socio-economic development, and effectively solve the "three rural" issue, improve the health of farmers.Since 2003 Jiangsu Province has 10 counties or cities across the province as the pilot areas of the new rural cooperative medical care, to the end of 2007, all cities and counties (cities, districts) the rate of joining this system has reached a combined rate of more than 95%. Basic reduced the problem of "poor for illness, illness returning to poor". However, in practice the concrete operation there are still many questions to be solved. The compensation mechanism constructions of the new rural cooperative medical system are the core issues and key aspects. It is directly related to the participation of farmers and the degree of benefit. It is also directly related to the sustained and healthy development of this system.Because of unbalanced regional economic development in Jiangsu Province, the operation and development of this new rural cooperative medical system shows different characteristics in southern, middle and northern part of the province. This paper conducted a comprehensive comparison on the compensation mechanism in the above menthioned three parts. The purpose is to promote the new compensation mechanism together to even better run this system. In this paper, Kunshan city of Southern Jiangsu, Taizhou city of middle Jiangsu and the North Jiangsu Ganyu were selected as a survey and interviews to carry out an empirical research on the compensation mechanism. The use of cluster random sample of households to visit questionnaire, and combined with the new agricultural-related personnel to carry out the interviews one by one. After treatment on the research data, we conducted a comparative evaluation of satisfaction and concluded that: the rank of the farmers'satisfaction toward the compensation mechanism is the highest in Southern, followed by the Northern areas and the worst is the middle part. In order to promote the new system in Jiangsu Province jointly balanced, sustainable development, policy recommendations were put forward for the region and the province.This article is divided into seven chapters: first chapter is the introduction. The main topics of this chapter describes the background, significance and the major research methods. Chapter two introcuces the evolution of this system, the different medical insurance types in other countries and how to learn form those types. Chapter three shows the present situation of the new rural cooperative medical compensation mechanism. Chapter four made vertical and horizontal comparisons of policies and compensation mechanisms of the new rural cooperative medical system in Ganyu county, Taixing city and the Kunshan city. On this basis, the fifth chapter makes a fuzzy comprehensive evaluation on the compensation mechanism in the three counties. Chapter six evaluates regional differences of compensation mechanisms and gives suggestions respectively and the last chapter makes policy recommendations to optimize the compensation mechanism for this system in Jiangsu provinces.
