Objective: To explore the changes of large arterial stiffness and cardiac structure in prehypertensive people. Investigate the the value of early diagnosis between arterial stiffness indicators to reflect prehypertensive vasculopathy.Methods: Screening health examinees in August 2007 - September 2008 in the third Xiangya hospital as research objects. Investigated case history, made physical examination and blood biochemical examination.Used arteriosclerosis diagnostic devices VP-1000 to determine brachial-ankle pulse wave velocity (BaPWV) and ankle-brachial index (ABI); Carotid ultrasonography determine inter-media thickness (IMT); Echocardiographic evaluated Left ventricular wall thickness and weight. Included 1454 cases of effective samples, stratifed with age, sex, body mass index and lipid levels respectively, selected prehypertension, high blood pressure and normal blood pressure each of 220 cases randomly for statistical analysis.Compared the changes in the indicators in prehypertention, analysis relevance btween Blood pressure and BaPWV, ABI ,carotid IMT.Results:1. BaPWV in prehypertension, high blood pressure and normal blood pressure group were (1390±171) cm / s, (1651±299) cm / s, (1308±145) cm/s, compared among the three groups were significantly different (P< 0.01), Prehypertensive group increased than in the normal group (P< 0.01), lower than the hypertensive group(P< 0.01). ABI were 1.115±0.060, 1.132±0.067, 1.114±0.061, prehypertensive group compared with normal blood pressure had no difference, lower than the hypertensive group (P< 0.01).2. CCA-IMT in prehypertension, high blood pressure and normal blood pressure group were (0.731±0.104) mm, (0.779±0.162) m, (0.725±0.102) mm. CCA-IMT in Prehypertension and normal blood pressure group had no statistical difference, high blood pressure group were significantly thickened (P<0.01). IMT thickening rate of prehypertension (41.8%) compared with normal blood pressure group (39.5%) showed no statistical difference, but lower than the hypertensive group (55.4%). 3. Prehypertensive group compared with normal blood pressure group, interventricular septum thickness (IVST), left ventricular posterior wall thickness (LVPWT) and relative wall thickness (RWT) increased (P < 0.05), left ventricular mass (LVM) and left ventricular mass index (LVMI) increased (P < 0.05), but left ventricular end-diastolic diameter (LVDd) and aortic root diameter (AO) in the two groups had no significant difference.4. Correlation analysis for indicators of arterial stiffness and blood pressure level showed that: BaPWV,ABI,CCA-IMT was significant positive correlation with the systolic pressure, diastolic blood pressure, pulse pressure and mean arterial blood pressure.5. BaPWV was positively correlated with CCA-IMT ( r = 0.271, P< 0.01), After adjustment of blood pressure the Correlation still exists(r = 0.156, P< 0.01, regression equation BaPWV = 1041.83 +549.15×IMT. ABI was positively correlated with CCA-IMT( r = 0.166, P< 0.01), After adjustment of blood pressure the Correlation still exists (r = 0.136, P P< 0.01), regression equation ABI = 1.059 +0.082×IMT.Conclusions:1. In prehypertension, arterial stiffness indicator BaPWV significantly increased, suggest the existence of early arterial stiffness changes.2. Compare with normal control, the left ventricular wall thickness and weight increased in prehypertensive people.3. BaPWV was positively correlated with carotid artery IMT, suggesting BaPWV can be simple and effective assessment indicator of arterial stiffness for prehypertension.
