Objective: To explore the emergent diagnosis,the time to be operation, the skills in microneurosurgery of intracraninal aneurysm, and the capability of doing operation directed by CTA.Methods: Our hospital did 324 intracranial aneurysms from from May 2006 to April 20009. During those 82 cases who had received emergent microneurosurgery were analyzed retrospectively. All cases of intracraninal aneurysm were confirmed through DSA or CTA exam and underwent microneurosurgical clipping . Postoperation DSA or CTA exam were executed on all patients .Results: All the 82 patients with aneurysm diagnosist by CTA or DSA did micro-operation during 72 hours after SAH.And,8 of them did during 6 hours ,while 19 of then did during 24 hours. It was proved that the diagnosis of the CTA before the operation was as the same as what was seeing during the operation.The CTA before the operation can advise the locations,size,shapes,directions,of the aneurysm,and can prove the relationship of the issue nearby .It can prove important evidence for emergent operations.All the 82 intracraninal aneurysm were totally clamped. No clinical deterioration or surgical mortality occurred in this series . Conclusions: DSA is most valuable for the diagnosis of the intracraninal aneurysm . And CTA is the most important examinations for early diagnosis and Microneurosurgery treatment of the intracranial aneurysms, it can even replace DSA in some conditions. Microneurosurgery treatment is the radical treatment for intracraninal aneurysm. Appropriate the suitable operative approaches and skilled microneurosurgical techniques based on the size, type, location and adjacent relationship are the keys of success in the treatment.
