Objective To investigate the related influential factors of postoperative in acetabulum fracture.Methods From January 2004 to June 2008,148 cases of all acetabular fracture patients which have been operated were collected in the department of orthopedics.135 cases were selected.The patients were 93 men and 42 women,with an average age of 41.26 years (range,17-72 years). Surgery curative effect are scored by the modified Merle d'Aubigne-Postel ratingacetabular fracture functions . According to surgical treatment might be affected a number of factors.10 factors such as age,gender,operative timing,operative approach,classification of fracture,diaplastic quality of fracture and heterotopic ossification so on were used as univariate and logistic multi-factor regression analysis.Results 135 cases patients were followed up at a mean of 28.5 months(6~60 months).Univariate analysis showed that classification of fracture, operative timing, diaplastic quality of fracture, whether or not any injury of the femoral head cartilage, whether or not intra-articular bone block and heterotopic ossification were significantly correlated with operative effect(p<0.05).Among 135 cases,excellent and good results were found in 113 cases(83.7 percent),fair and poor in 22 (16.3 percent) by the modified Merle d'Aubigne-Postel rating acetabular fracture functions. The result of the functional excellent and good rate were respectively for 95.3 percent and 78.3 percentin the simple type and the complex type fracture group(p=0.012).Within 14 days of surgery and more than 14 days for the surgery group were respectively for 89.0 percent and 61.5 percent(p=0.001).Anatomical reduction,satisfied and not with the reduction group were respectively for 92.4 percent,82.0 percent and 57.9 percent(p=0.001).Whether or not any injury of the femoral head cartilage group were respectively for 88.8 percent and 70.3 percent(p=0.009). Whether or not intra-articular bone block were respectively for 89.7 percent and 68.4 percent(p=0.003). Whether or not heterotopic ossification were respectively for 87.1 percent and 63.2 percent(p=0.009).Logistic multi-factor regression analysis show operative timing and diaplastic quality of fracture were two main factors for postoperative effect in acetabular fracture.Conclusion Operative timing and diaplastic quality of fracture might be the independent risk factors of postoperative functional outcome in acetabular fracture. Diaplastic quality of fracture was one of the most important controllable factors.
