PARTⅠTHREE-DIMENSIONAL ANATOMICAL EVALUATION OF THYROID ARTERIES WITH 64-SLICE HELICAL COMPUTED TOMOGRAPHIC ANGINOGRAPHY0bjective: To evaluate the ability of 64-slice helical CT angiography (CTA) in identifying and demonstrating the origins and courses of thyroid arteries by using the three-dimensional reformation technique.Methods: One hundred and three cases were performed neck CTA with a 64-slice scanner. Three-dimensional images of thyroid arteries were processed on AW 4.2 workstation. Spatial anatomical characters of the thyroid arteries using volume rendering (VR), maximum intensity projection (MIP), multiplanar reconstruction (MPR), and advanced vessel analyze (AVA) were observed.Results: Two hundred and seven superior thyroid arteries, one hundred eighty inferior thyroid arteries and ten ima thyroid arteries were clearly displayed in One hundred and three cases. Both two sides'superior thyroid arteries mainly appeared to originate from the initiation of the external carotid artery (61.35%); two sides inferior thyroid arteries mainly from the thyrocervical trunk (87.78%), and the ima thyroid arteries from brachiocephalic trunk (88.89%). Both the two sides superior thyroid arteries were mainly arose from antero-endhymenine wall of genesis arteries (58.45%), inferior thyroid arteries of the thyrocervical trunk from endhymenine wall, and the ima thyroid arteries of genesis arteries all from anterior wall. There were seven glandular organ artery of superior thyroid artery distribution patterns with the L1R1 predominant (37.25%), five non-glandular organ artery of superior thyroid artery distribution patterns, and three kind branches (laryngeal artery, cricothyroid artery and sternocleidomastoid artery), and two distribution patterns of inferior thyroid glandular organ artery.Conclusion: The thyroid artery anatomy was complicated, and 64-slice helical CTA could clearly visualize the features. PARTⅡPRELIMINARY RESEARCH AND CLINICAL VALUE OF FEEDING ARTERIES IN PATIENTS WITH THROID CANCER USING 64-SLICE HELICAL COMPUTED TOMOGRAPHIC ANGINOGRAPHY0bjective: To investigate the three-dimensional imaging characters of the feeding arteries in patients with thyroid cancer and the clinical value using 64-slice helical CT angiography.Methods: The scan data of 101 cases (normal group, 65 cases and thyroid cancer group, 36 cases) that consistent with this investigation and received neck 64-slice CTA were processed on AW 4.2 workstation. Spatial anatomical characters of the feeding arteries in patients with thyroid cancer were observed and analyzed using volume rendering (VR),maximum intensity projection (MIP), multiplanar reconstruction (MPR) and advanced vessel analyze (AVA).Results: The inner diameters of normal group are (1.4~2.7) mm, mean value (1.78±0.25) mm and (0.8~2.9) mm, mean value (1.66±0.37) mm. All the feeding arteries of tumor in thyroid cancer group were thyroid arteries (76 rami). Three-dimensional imaging characters of these feeding arteries such as origins, numbers and inner diameters could be showed exactly. The feeding arteries of thyroid cancer are thickening, squirm, and there are rambling vascular nets in affluent blood intratumorally of the thyroid cancer group. The resource of feeding arteries were statistics 3 patterns: merely superior thyroid artery feeding pattern 18 cases (50%), merely inferior thyroid artery feeding pattern 6 cases (16.67%) and both superior and inferior thyroid arteries feeding pattern 12 cases(33.33%).Conclusion: 64-slice helical CTA not only can supply detailed data of the feeding vessels of thyroid cancer including their origin and number, but also can visualize these vessels clearly. It plays a very important role in the treatment of thyroid caner.
