【Background】Cerebral infarction is one of the major diseases threat to people's health in our country , only a very small number of people can benefit from long-term anti-platelet aggregation drugs .There may be have more complex mechanism in the formation of thrombus. Microparticles derived from vascular endothelial cells or peripheral blood cells carrying phosphatidylserine , Which have procoagulant potential, may play an important role in cerebral infarction.【Objective】To investigate the level of CD41~+PS~+PMP and CD62e~+PS~+EMP in the evaluation of the extent of cerebral infarction patients, and analysis the relationship between the level of CD41~+PS~+PMP and CD62e~+PS~+EMP and other clinical parameters.【Methods】Choose 43 cases of acute cerebral infarction patients and 20 healthy people,detect the level of CD41~+PS~+PMP or CD62e~+PS~+EMP in peripheral blood with flow cytometry, and record the infarct volume from head CT and location of lesion, determination neurological deficit scores, observation different time from the disease can bring the different of the level CD41~+PS~+PMP or CD62e~+PS~+EMP level and to explore its clinical significance. At the same time, all patients are rocord the blood pressure, random plasma glucose, cholesterol, triglycerides, blood coagulation test and analysis of blood cells. (1) According to the size of infarct volume ,43 patients had divided into small infarction group, middle infarction group,large infarction group, according to location of Lesion they were divided into cortex and with subcortical infarcts group, subcortical infarction, brainstem or cerebellar infarction groups; also they were divided into mild group, moderate to severe group according to the NIHSS scores. From the disease incident to the time of collect blood sample they were divided into 24h group and 24-72h group. (2) respectively, analysis the level of CD41~+PS~+PMP or CD62e~+PS~+EMP with other parameters,such as age, systolic blood pressure, diastolic blood pressure, cholesterol, triglycerides, random plasma glucose, fibrinogen, absolute platelet number,if they existence correlation relations ,if have correlation then analysis of covariance re-inspection of the parameters are related to judge which play a major role in the impact of the level of CD41~+PS~+PMP and CD62e~+PS~+EMP.Finally ,all parameters are related to the level of CD41~+PS~+PMP and CD62e~+PS~+EMP then progressing multiple stepwise regression, analysis whether they also can play a role in the multi-factor analysis.【Results】1. In the acute phase of cerebral infarction ,the level of CD41~+PS~+PMP and CD62e+ PS+EMP were significantly higher than the healthy control group (P <0.01);2. CD41 + PS+ PMP, respectively, compare three different infarct volume groups,they has statistically significant (P<0.05) compare each other. Yet, CD62e~+PS~+ EMP ,compare large volume group to middle or small volume group has statistical significance(P<0.05),then, compare middle volume group to small volume group has no statistical significance(P>0.05),but has the trend of elevated.. there have no statistical significance(P>0.05) in different location of Lesion for the level of CD41~+PS~+PMP and CD62e~+PS~+EMP. Compare Moderate to severe stroke group to the mild stroke group,the level of CD41~+PS~+PMP and CD62e~+PS~+EMP are higher,and have statistical significance(P<0.05). There was no significant difference(P>0.05)of the level of microparticles between 24-72h group and 24h group.3. The level of CD41~+PS~+PMP level correlated with the NIHSS score, systolic blood pressure, lesion volume, absolute white blood cell numbers in multiple stepwise regression analysis, and NIHSS scores are the main influencing factor, the level of CD62e~+PS~+EMP also correlated with lesion volume, random plasma glucose. 【Conclusion】1. Compare to healthy control group, acute cerebral infarction group is observed the elevation of the CD41~+PS~+PMP and CD62e~+PS~+EMP.2. The level of CD41~+PS~+ PMP and CD62e~+PS~+EMP separately independent positive correlation with the scores of neurological deficit and infarct volume, can better reflect the extent of cerebral infarction patients .
