Objective:Stem cells are special cells which have some capacity for self-renewal and the potencies to differentiate with multi-direction. Under normal circumstances , Stem cell self-renewal and differentiation ability were regulation and control strictly. Once such a mechanism is damaged, Cells will be unlimited growth, reproduction and formation tumor. Although the complex tissue type of mammary tumor , they all stem from mammary stem cells, Breast cancer stem cells were extensive attention as the first cancer stem cells were identified in solid tumors.As other cells, breast stem cells are also multi-potential, they can generate progenitor cells at different level. and differentiate into myoepi- thelial and ductal finally. Though breast stem cells exist in breast tissue. special marker of mammary stem cell still need unceasing exploration. To a- certain extent, cytokeratin expression in mammary gland respond the cell differentiation. CK19 is one of keratins which expresses most early in embryo. Tt often expresses in mammary gland Terminal Duct Lobular (TDLU), Masculine gland epithelial cell with CK19 positive were existence in fetal breast.In our research, we collected the mammary tissue of operation and detect the expression of CK19, cyclinD1/CDK4 and NF-κB in 10 case of normal mammary tissues, 17 cases of benign ductal hyperplasic, 38 cases of invasive ductal cancer, in order to investigate the role of the factoring the differention and development of human breast stem cells, and reveal the rule and mechanism of differention and malignant transformation.Methods:1. 10 cases of normal mammary gland tissues, 17cases of mammary gland proliferated diseases, 38cases of mammary gland tumors were inve- stigated by using S-P method of immunohistory and analyzed the exp- ression of CK19 in each tissue. And their differences in the types of normal, proliferated tissues, differentiated extent and respective dependability were also studied.2. 10 cases of normal mammary gland tissues,17 cases of mammary gland proliferated diseases, 38 cases of mammary gland tumors were inve- stigated by using SP method of immunohistory and analyzed the expression of cyclinD1/CDK4 and NF-κB in these tissues. And their differences in the types of normal, proliferated tissues, differentiated extent and respective dependability were also studied.Results:1. CK19 expressions were detected in normal mammary gland tissues, proliferated and tumor tissues, data showed significant difference between the expressions of normal tissues and infiltrating carcinoma tissues and significant deviation existing in expressions of normal as well as prolix- ferated tissues and tumor tissues(P<0.05).2 There is no expression of cyclinD1 in normal mammary gland tissues and 11.8% hyperplasic tissues express cyclinD1, in the breast carcinoma tissues was observed in 14 of 38(47.4%) cases, significant difference was found in normal as well as proliferated tissues and tumor tissues(P<0.05).3. The expression of CDK4 protein in normal mammary gland tissues, proliferated and tumor tissues were10.0%, 17.4% and 45.9%, it showed significant deviation existing in expressions of normal as well as proli- ferated tissues and tumor tissues(P<0.01).4. The expression of NF-κB in normal mammary gland tissues, prolix- ferated and tumor tissues were 20.0%, 35.3% and 66.0%, it showed signi- ficant deviation existing in expressions of normal as well as proliferated tissues and tumor tissues(P<0.05).5. Spearman's rank correlation test showed that CK19 expression in breast carcinoma had positive correlation with cyclinD1 expression. Corre- lation coefficient: r=0.535, P=0.038. CK19 expression in breast carcinoma had positive correlation with CDK4 expression. Correlation coefficient: r=0.821, P=0.00. cyclinD1 expression in breast carcinoma had positive correlation with NF-κB expression. Correlation coefficient: r=671, P=0.00.Conclusion:1. The significant differences between expressions of CK19 implied that there were"stem-like cells"existing in the tissue of breast tumor.2. CK19 is probably relevant with differentiated extent and it might be an important index for evaluating the prognosis of breast tumor.3. CyclinD1/CDK4 expressions were detected in normal mammary gland tissues, proliferated and tumor tissues, data showed significant difference between the expressions of normal tissues and infiltrating car- cinoma tissues. The results from correlation analysis showed that there was strong relation between the expression of CK19 and cyclinD1/CDK4, all the outcomes pointed out that cyclinD1/CDK4 gene had some effect to the stem cell differentiation.4. The expressions of cyclinD1/CDK4 and NF-KB were detected in various mammary gland tissues, datas showed significant positive correla- tion between them, which implied that during the canceration of mammary stem cells, the overexpression of NF-κB may lead to the accelerated expres- sion of cyclinD1/CDK4.
