Objective: To evaluate the clinical value of the inter-and intraventri- cular asynchronous in patients with congestive heart failure(CHF) by tissue tracking imaging(TTI)and tissue synchronization imaging(TSI).Methods : Thirty-five controls and thirty patients with congestive he- art failure (CHF) were studied by means of TTI to analyze their systolic displacement curve of the left ventricular (LV) walls and the basal segment of right ventricular lateral. The time from the beginning of the QRS compl- ex to peak myocardial systolic displacement(Ts)and the peak systolic displacement (Ds) were measured. The maximal difference in Ts of 12 LV segments (Max-â–³Ts), the differences in Ts within the same wall of 2 seg- ments (Ts-diff) and the mean of Ds (mDs) were calculated. The basal segm- ent of right ventricular lateral and 12 LV segments'contraction.Time to peak (Tp)and peak velocity (Vp) were measured by TSI. The difference in Tp within the same wall of 2 segment (Tp-diff),the stand deviation of time to peak (Tp- SD) and the mean of Vp (mVp) were calculated. The difference of Ts and Tp between the basal segment of left and right ventricular lateral were measureed as the left and right ventricular delay (L-RV-D-Ts,L-RV-D-Tp).The parameters of LV pump function included the ejection fraction of LV (LVEF) and Tei index were measured.Results: (1) Max-â–³Ts, Ts-diff,Tp-SD,Tp-diff and L-RV-D-Ts , L-RV- D -Tp in CHF group were significantly prolonged, mDs ,mVp and LVEF,Tei index obviously descended when compared with control group (all P<0.01). (2) In CHF group , Max-â–³Ts was negatively correlated with LVEF and mDs (r=-0.68,P<0.01;r=-0.59,P<0.01) and positively with Tei index (r=0.64, P<0.01); Tp-SD was negatively correlated with LVEF and mVp (r=-0.63,P<0.01;r=-0.67,P<0.01) and positively with Tei index (r=0.66,P<0.01),but they were not correlated with QRS duration. MDs and mVp were correlated positively with LVEF (r=0.61,P<0.01; r=0.69,P<0.01).Conclusions: Tissue tracking imaging and tissue synchronization ima- ging can evaluate the inter-and intraventricular asynchrony noninvasively and quantitatively, while it is correlated positively with LV function. TTI and TSI maybe better supervise CRT efficently.
