Background:The lung cancer is the one of most common malignant tumors,the present disease incidence rate still year by year rose,also when the majority of patients diagnosis has been the later period.The lung cancer divides into the non-cellule lung cancer(NSCLC) and the cellule lung cancer(SCLC),The non-cellule lung cancer(NSCLC) mainly includes the squamous-cell carcinoma,the adenocarcinoma and the undifferentiated carcinoma.Generally speaking the cellule lung cancer is accompanied by the mediastinal lymph node metastasis. Once diagnoses,the most patients will accept the chemotherapy and the radiotherapy.But the treatment of the non-cellule lung cancer take the surgery excision as the foundation, the union radiotherapy and the chemotherapy.Because the histological type is different,its treatment measures is also different.How to enhances the early diagnosis rate of the lung cancer and make clear the histological type,has the extremely vital significance to the treatment.But,any one detecting means at present can not achieve.Therefore through many kinds of ways, multiple layers to examine the lung cancer patients have the vital clinical significance.The research discovered that the tumor produces many kinds of tumor markers picked out in the tumor patient′s body fluid in the deve- loping process,which are produced few or not in the normal tissue.The tumor patients blood and pleural effusions or ascites are examined tumor markers,which are very important for the discovery of the tumor.But there is no one special tumor marker which can diagnose the carcinoma at present. Many items of the tumor marker examination can make up the insufficiency of the sole examination.Many items of the tumor marker examination identify benign and malignant hydrothorax also widely to utilize in clinically at home and abroad. Although excel the sole examination,Many items of the tumor marker examination have more combination regimen, report inconsistent in specificity and sensitivity. NSE has higher specificity for the SCLC.CEA has the very high clinical value for the diagnosis of lung adenocarcinoma.CYFRA21-1 has important diagnostic significance for lung squamous cell carcinoma.Therefore we explore the clinical diagnosis value of tumor marker:CEA,NSE,CY21-1,GLU which apply to the Clinic in tuberculous effusion(TE) and malignant pleural effusion(MPE).Objective:To explore the clinical diagnosis value of CEA,NSE,CY21-1?GLU(four markers) in tuberculous effusion(TE) and malignant pleural effusion(MPE).Methods: To collect data from 96 cases of pleural effusions between January,2004 and October,2008.To detect the levels of four markers were examined in 42 cases of malignant pleural effusion(MPE) and 54 cases of tuberculous effusion(TE).Results:The levels malignant group of four markers in pleural effusion were higher than the TE(P < 0. 05).The sensitivity is: 64.3%,26.2%,57% respectively,Veracity is:94.4%,94.4%,80.4%;Combined four detections in pleural effusion,The sensitivity is 90.5%,vearcity is 83.3%.Conclusion:The combined detection of four markers in TE and MPE have significant value.
