Objective1. To study the anatomic structure of operative approach of OCM Minimal Incision Replacement of Total Hip, definite the safety range of OCM Minimal Incision Replacement of Total Hip.2. Reconstitute the CT three-dimensional image of hip joint. So as to provide imaging reference for the design of hip prosthesis and hip operation.Material and Method1. Macromicrodissection: to dissect the hip joint according to the layer in OCM operating approach with 16 hip joint specimen; observe the vessel and nerve of the hip joimt and measure the correlated date; explose the the ligment of the hip joint, measure the length and thickness.2. Blood vessel proplasm: to observe the distribution, branch and anastomosis of Ligament of hip joint and measure the length thickness of the main ligaments with a hip joint specimen.3. Angiography 3D reconstruction: to scan a fresh dead body specimen on CT with a radiopaque perfusing material consisting of carboxymethylcellulose-suspended lead oxide, reconstitute and treat he CT three-dimensional image of hip joint and probe it's application with a hip joint specimen that it's ligament is dissected.4. Statistical treatment: to statistics data with SPSS13.0.Resluts1. The instance between the anterior border of gluteus medius and anterior superior iliac spine is 4.27±0.24cm, the instance between the lateral most outstanding point of greater trochanter and the point that the superior gluteal nerve cuts through the gap of gluteus medius and gluteus minimus: man is 8.36±0.74cm, woman is 7.39±0.50cm;the instance between the nterior superior iliac spine and the point that the superior gluteal nerve gets into the tensor fasciae latae: man is 6.64±0.47cm, woman is 6.25±0.34 cm. The included angle between the anterior border of gluteus medius and femoral major axis is 29.5±2.0 o.2.The length of Bigelows Y-ligament: superior branch is 4.01±0.65cm, inferior branch is 5.36±0.52cm, the hickness: superior branch is 0.83±0.12cm, inferior branch is 0.56±0.14cm; the length of interpubic ligament is 2.84±0.42cm, the hickness is 0.25±0.16cm; the length of ischiocapsular ligament is 2.32±0.45cm, the hickness is 0.48±0.13cm.3.The reconstituting CT three-dimensional image of hip joint is very clear and demonstrates that the blood vessel wall is quite smooth and glossy, we can discern the shape of blood vessel and ligaments of hip joint.Conclusion1. The OCM operative incision is on the line between greater trochanter and the point which is 4.27±0.24cm behand the anterior superior iliac spine. From the greater trochanter, the length of the incision: man doesn't exceed 8.36±0.74cm; woman doesn't exceed 7.39±0.50cm. this is the operative safety zone.2. the reconstructed images could be arbitrarily revolved, dissected, selectively shaded and demonstrated the cross section distribution of the vessel and nerve, we can not only arbitrarily measure, revolve, cut, recombine and panotograph the structure of soft tissue and sclerotin of hip joint, but also make the operating simulation and navigation, reduce or avoid the happening of all sorts of operating complication.
