| PrefaceAs a common complication of chronic kidney disease,Renal parenchymal hypertension is an important risk factor leading to kidney function and complicating cardiovascular disease.Compared with the same level of primary hypertension, fundus changes of renal parenchymal hypertension is more serious,cardiovascular and cerebrovascular complications is more,and more easily progresses into malignant hypertension.In clinical treatment,blood pressure of renal parenchymal hypertension is more difficult than primary hypertension.Barnidipine hydrochloride is a new long-acting dihydropyridine calcium channel blocker,is the world's first single optical isomers.In this paper,it observed clinical effect of hydrochloride barnidipine treatment of renal parenchymal hypertension.Provided an important way to treatment of renal parenchymal hypertension.Experimental Data1.SubjectsThe subjects in the study were 54 patients with renal parenchymal hypertension, the subject criteria was the patients diagnosed as renal hypertension by the urine examination,renal biopsy,or imaging.28 males and 26 females,age range 20-65 years old.All subjects stopped other anti-hypertensive drugs during the period of observation.This study carried on an open,self-control study.2.Drugs and equipmentDrugs:Barnidipine Hydrochloride(Hypoca):from Ansi Tai Lilly(China) Limited.Blood biochemical testing equipment is flow cytometry(Heath AMK XE-2100);multi-function automatic biochemical analyzer(Olympus AV-640).Experimental Methods1.All selected objects were in the treatment period directly,treatment period was 8-week.patients who intaked calcium channel blocker changed to intake co-Puka 15 mg / day as a treatment drug.patients who intaked ther antihypertensive drugs except calcium channel blocker changed to intake co-puka 10mg / day,those who responsed to the intake of drugs for 2 weeks continued to intake the original dose, those who did not response to the intake of drugs for 2 weeks increased the amount to 15 mg / day.All selected objects during the treatment(0 weeks,2 weeks,4 weeks, 6 weeks,8 weeks and above) recorded blood pressure,heart rate,blood routine test and blood biochemical test.2.Detection Method:blood pressure measured by the same person,by the same hemomanometer at least 3 times.Heart rate measured by the same echometer in 1 minute.blood routine test by flow cytometry;liver function,renal function,blood sugar,blood lipids by wet chemical method,by multi-function automatic biochemical analyzer.3.Statistical analysis:All statistical analysis used SPSS 13.0 software. Measurement data mean±standard deviation(-x±s),paired t test was used to statistical analysis before and after treatment.P<0.05 was considered significant difference.ResultsIn the end of treatment with Hydrochloride bamidipine,the blood pressure decreased compared with before treatment,the difference was highly significant(P<0.001 ).Heart rate declined slightly compared with the previous level,the difference was significant(0.02<P<0.05).Blood routine test and biochemical indicators and pre-treatment had no significant difference compared with before treatment(P>0.05 ).Discussion renal parenchymal hypertension is a common complication of chronic kidney disease patientsmorbility accounts for the first one in the secondary hypertension,is an important risk factor leading to cardio-cerebral vascular accident,timely and effective control of blood pressure is a prevention and treatment of chronic kidney disease.In recent years,a large number of studies have shown that long-acting calcium antagonists can reduce the cardiac load and myocardial metabolism,improve left ventricular diastolic function,and can inhibit inflammatory mediators,reduce free radicals,inhibit platelet activation and platelet accumulation,significantly improve endothelial function,effectively protect the kidney function,in addition long-acting calcium antagonists can effectively protect the brain cells,reduce the incidence of stroke.Bamidipine hydrochloride is a new long-acting dihydropyridine calcium channel blocker,is the world's first single optical isomers.The mechanism via cell-specific transmembrane potential-dependent calcium ion channels,inhibited calcium into cells,relaxs selectively peripheral vascular and coronary vascular smooth muscle. Animal experiments confirm that this preparation dialate coronary artery,vertebral artery,femoral artery and other blood vessels,increase blood flow to organs,inhibit renal hypertension and vascular disease.In addition,the preparation reduced systolic blood pressure in rats slowly,dialated remarkablely the kidney efferent and afferent arterioles,increased significantly glomerular blood flow.Compared with the role of traditional medicines singlely,Barnidipine Hydrochloride for antihypertensive effect not only had the role of the characteristics of strong and long duration,but also had the unique role of increasing renal blood flow,dialating the renal efferent and afferent arterioles,reducing glomerular pressure,diuretic,natriuretic,and inhibition of renal vascular hypertensive disease occurrence and development.The metabolic product excreted mainly by the faeces and urine,There is no prototype drugs in the urine,which more in accordance with the modern science concept of treatment for renal hypertension.Our research showed that hydrochloric acid treatment for renal parenchymal hypertension bamidipine was effect,safe and reliable.Provided an important way to the treatment for renal parenchymal hypertension.Conclusion1.In the patients after receiving barnidipine hydrochloride treatment,blood pressure decreased significantly than before.Therefore,the antihypertensive effect of Barnidipine Hydrochloride was significant.2.In the patients after receiving bamidipine hydrochloride treatment,compared with those before treatment,there were no serious adverse events,blood and biochemical indicators had no difference.So hydrochloride barnidipine buck was safe. |
