The Study On Relief Incision And Anastomosed Of Internal Anal Sphincter In Low Anal Fistula

Background: Anal fistula is a frequent disease often running a chronic course. The most common procedure for transsphincteric anal fistula is anal fistulotomy. Although it is generally considered unlikely for incontinence to develop if the anal sphincters are divided below the dentate line,impaired continence occurred in 18 to 52 percent of patients after anal fistulotomy.The relief incision and stitch of internal anal sphincter is a new technique which can avoid those side effects.It will be an alternative for low anal fistula.Objective: For the purpose of obtaining a proper procedure of protecting internal sphincter in the operation for anal fistula diseases to raise the curative effects,the relief incision and anastomosed of internal anal sphincter was designed for application in the treatment of corresponding low anal fistula. Method: 18 patients with low anal fistula were included in the study,of which 9 cases underwent the relief incision and stitch of internal anal sphincter(the study group),while 9 cases underwent fistulotomy(controlled group).There was no difference between the groups in age, sex distribution, or Follow-up was by call questionnaire inquiring about fistula recurrence, incontinence, general curative effect, anal deformation and the healing time .Results: There was no difference in the general curative effect, healing time, recurrence. But complications such as cicatrical deformity,underwear staining and difficulty holding gas of two groups showed marked difference. Concludes: Both techniques are equally effective in eradicating the low fistula, the anal function are protected better with fistulotomy.
