Clinical Retrospective Analysis Of ODS (Analysis Of Seven Cases)

Objectives: To investigate the clinical features /neuroimaging features /treatment and prognosis /of osmotic demyelination syndromeMaterials and Methods:Analyze the 7 cases with osmotic demyelination syndrome that has been diagnosed in the cerebrovascular department of qilu hospital. The 7 cases consist of 1 patient with CPM and EPM , 2 patients with CPM, 4 patients with EPM. The clinical features of 7 patients with osmotic demyelination syndrome were analyzed, including the inducing factors, clinical manifestations, cranial MRI, treatment and prognosis.Results: All the seven patients had the history of rapidity of correctingchronic hyponatraemia. 6 patients has change of consciousness and new neural signs, and 1 patinet dosen't have change of consciousness but has new neural signs.1 patient suffers from dysarthria, 3 patients suffer from dysphagia , 1 patient suffers from parkinsonism , 2 patients suffer from epilepsy . By means of MRI scan ,2 patients were confirmed as central pontine myelinolysis and 4 patents as extrapontine myelinolysis and 1 patient as central pontine myelinolysis and extrapontine myelinolysis . 6 patients recover well and 2 of them can live indenpendtly. 1 patiet give up treatment and die after he leave the hospital .Conclusion: The occurrence of hyponatremia and its too rapid correction may be associated with the onset of CPM/EPM. CPM/EPM is not a deadly disease, which can be cured if properly treated. The increase in knowledge about myelinolysis in central pontine and extrapontine is essential,which can help us find this disease in time .And avoiding the rapidity of correcting chronic hyponatraemia is the key point for preventing its happening. The prognosis of CPM/EPM dosen't depend on the severity of MRI investigation or neuroal injury . To early use glucocorticoid and high pressure oxygen can improve prognosis. So when we correcte the hyponatremia , we should always carry out neural physical examation .When we find abnormality ,we should carry out imageological examation.
