Background:Hilar cholangiocarinoma(HCCA) was an uncommon disease,but its incidence increased year by year.Because of special anatomical location and biology particularity,the onset of HCCA is latent and clinical symptoms at the early stage are non-specific,the early diagnosis is hard and patients are almost in late stage. In recent years,resection of HCCA is expanding constantly in order to improve the resection rate and treatment results.Porto-enterostomy makes it possible for extensive resection and biliary drainage.The present research is to retrospectively analyze the clinical materials of 10 patients with advanced hilar cholangiocarinoma who were treated between March 2005 and April 2008 and to evaluate the effect ofporto-ente-rostomy for patients with advanced hilar cholangiocarcinoma.Objective:To present the experience of surgical treatment of 10 patients with advanced hilar cholangiocarinoma.To evaluate the porto-enterostomy for patients with hilar cholangiocarcinoma.Methods:A retrospective analysis was made on the clinical materials of 10 patients with advanced hilar cholangiocarinoma who were treated by tumor resection and porto-enterostomy between March 2005 and April 2008.Results:All the 10 patients were treated by hilar cholangiocarinoma resection and Roux-en-Y porto-enterostomy.None of them had biliary fistula.Hemobilirubin levels decreased to normal level in 26-35 days after the operation.Transaminase decreased significantly,and was nearly normal at discharge.All the patients resumed normal life in 1 month after operation.The pathological results were as follows: papillary carcinoma 1 case,well differentiated adenocarcinoma 2 cases,moderately differentiated adenocarcinoma 4 cases,poorly differentiated adenocarcinoma 3 cases.Conclustion:Porto-enterostomy is a useful treatment of the advanced hilar cholangiocarinoma.
