Effect Of Different Dosage Of Simvastatin On Plasma Adiponectin In Type 2 Diabetes

Objective: To investigate the effect of different dosage of simvastatin on plasma adiponectin in Type 2 Diabetes (T2DM).Method: This was a prospective randomized controlled study. Sixty T2DM patients matched by age, sex, duration of diabetes, body mass index(BMI) and lipid-regulating drugs naive were enrolled. They were randomized into large dosage group (L group, Simvstatin 40 mg/d) and small dosage group (S group, Simvstatin 20 mg/d). The following data, adiponectin, hsCRP, fasting glucose, fasting insulin, lipids, blood pressure and BMI, were collected before and after 3months and 6 months of simvastatin medication.Results:(1)The level of Adiponectin didn't change significantly in L group and S group(P>0.05);also no difference between L and S groups(P > 0.05).The level of Total Cholesterol and Low density lipoprotein-Cholesterol(LDL-c) decreased after simvastatin in L and S groups(P<0.05). HOMA-IR increased in L group after treatment(P< 0.05). (2) According to the level of baseline LDL-c concentration, all patients were divided into two sub-groups, target reached group (LDL-c≤2.5mmol/L, n=20, 9 in L group, 11 in S group) and target not reached group(LDL-c>2.5mmol/L, n= 40, 22 in L group, 18 in S group). No statistical differences of adiponectin in each sub-group , and between L and S groups(P>0.05). (3) According to the level of baseline hsCRP, patients were divided into low-risk of cardiovascular disease group (hsCRP<1mg/L, n=32, 17 in L group, 15 in S group) and mid-high-risk of cardiovascular disease group (hsCRP≥1mg/L, n=28, 14 in L group, 14 in S group), No statistical differences in each sub-group , and between L and S groups(P>0.05).(4)Adiponectin was positively related with age, HDL-c(P<0.01),and negatively related with BMI(P =0.032).Conclusion: 1. There was no obvious effect on plasma adiponectin after Short-term simvastatin treatment in T2DM, but large dosage simvastatin may increase insulin resistance in some T2DM.2. Adiponectin was positively related with age and HDL-c, but negatively related with BMI.
