Objective:To explore the causes and related factors of the complications associated with the anterior approach of lumbar spine surgery.These results provide considerable evidences of how to avoid complications during anterior surgery of the lumbar spine.Methods:A retrospective analysis about 537 cases which had been carried out anterior approach surgery of the lumbar spine from May 1995 to December 2007 in the Second Xianya Hospital was performed.These cases include tuberculosis,oncosis,fracture,discitis,slippage,anomaly of lumbar spine.Gender,ages,entity,segment,typing of fracture, opportunity,length of stay,the cause of injure,the gra(?)e of Frankel, nut-locking device,length of operation,operative approach,volume of blood transfused and lossing,complications,etc.are statisticed.These information were recorded in standardized database,and analyzed by SPSS 11.0.Results:The research found out 119 cases and 152 times including 25 types complications related on anterior approach surgery of the lumbar spine had been taken place in 537 cases.The incidence of disease is 22.2%,the incidence rate is 28.3%,and the death rate is 0.37%.The rate of main complication is 6.5%,and the rate of secondary is 21.8%.The rate of common complication is 4.10%,and the rate of complication associated with the anterior approach of lumbar spine surgery is 24.2%. The incidence rates of complications on thoracolumbar were greater than these on lumbar.The incidences of complications related on fracture were greater than those related on tuberculosis,discitis,olisthe and anisotrophy. The incidence rate of main complication on fracture and oncosis was greater than others.The incidence rates of retroperitoneal hematoma and the injury of incomplete lumbosacral plexus were greater than others.Conclusion:The anterior approach of lumbar spine surgery is safely active modus operandi.The approach can treat affection on anterior lumbar.Though various complications are existed,it doesn't mean we doubt this therapeutic regimen.A lot of complications can be avoided,if treatment group have expert operation technology,opography is clear, and treatment after operation is correct.With improvement of surgical technic and development of Computer Aided Surgery,complications associated with anterior approach of lumbar spine surgery will be decreased sharply and patients will obtain the best cure.
