Object: Non small cell lung cancer is very malignant carcinoma. Most patients are at the stage of advanced NSCLC when first diagnosed. Even given chemotherapy ,the lung cancer mortality is very high and has bad prognosis .Among large of affecting factors, anemia and increasing platelet count are known as bad prognostic factors in the world. Whether stability of peripheral blood cell counts affects advanced NSCLC patients chemotherapy prognosis has been not reported. Our study will investigate the peripheral blood cell counts Influencing on chemotherapeutic prognosis in advanced non-small cell Lung cancer.Methods: Choose patients from Nov. 2004 to Nov.2006, diagnosed advanced non-small cell lung cancer by histology or cytology and staged by staging examinations(stage III, 32,stage IV, 55;squamous cell carcinoma,25,adenocarcinoma,58 and untyped carcinoma,4) and give at least 2-cycle chemotherapy with platinum-based associated with the third anti-tumor drugs regimen . Measure the counts of peripheral white blood cell, red blood cell, blood platelet, neutrophilic granulocyte and lymphocyte during the week before and after chemotherapy. Evaluate the response rate within four weeks and observe asymptomatic survival time and survival time. Taking Kaplan-Meier method and Cox multiple-factor regression model analyzed influencing on chemotherapy response rate and survival attributed to the stabilities of peripheral white blood cell, red blood cell, blood platelet, neutrophilic granulocyte and lymphocyte.Results: 87 patients are enrolled, and response rate is 26.4%. Till observing deadline, mortality is 82.8%.Median asymptomatic survival time is 5 months and survival time is 15 months.there is no difference on response rate and asymptomatic survival time between S≥1 and S< 1 (S is count before chemotherapy/count after chemotherapy) (P<0.05) .The patients (neutrophilic granulocyteS < 1) survive longer than those S< 1 (median survival time is 17 months:8months,Pis 0.002,<0.05 ) .Conclusion: Stability of peripheral blood cell count doesn't influence on chemotherapy short-term response rate in advanced non small cell lung cancer . Single factor analysis shows chemotherapy regimens and Stability of peripheral neutrophilic granulocyte are related with prognosis of advanced non-small cell lung cancer. The multiple-factor analysis shows that stability of peripheral neutrophilic granulocyte is an independent prognostic factor in advanced non-small cell lung cancer. Count declining patients were likely to survive longer than those with stable peripheral neutrophilic granulocyte count. This can help evaluating advanced non-small cell lung patients chemotherapy prognosis.
