Objective: To evaluate whether the changes of serum interleukin- 18 and sCD40L in Patients with acute coronary syndrome(ACS) correlated with severity of the diseases and whether interleukin-18 and sCD40L are reliable biochemical markers correctly indicates the severity of ACS.Methods: 71Subjects from Cardiology Department of the first Hospital Affiliated to Nanchang University, from Jan 2008 to Mar 2009 . In this study the spectrum of ACS only included Unstable angina pectoris(UA) and acute myocardial infarction(AMI).Patients with UA were separated into 3 groups by Braunwald asses and those with AMI were separated into 4 groups by Killip classes.The concentrations of serum inierleukin-18 and soluble CD40L were measured by enzyme linked immunoabsorbent assay.The coronary artery lesions score was recorded according to single,double and triple vessel lesions and Gensini scoring system.The relationships between interleukin-18 and soluble CD40L levels and the coronary artery score(the severity of coronary heart disease) were assessed.Data was estimated by SPSS13.0.Result :The levels of interleukin-18 and soluble CD40L were obviously higher in the AMI group and UA group than in the control group(P<0.01).The two markers were significantly higher in the acute coronary syndrome group than in the SA group(P<0.01) , The levels of interleukin-18 and soluble CD40L were higher in the AMI group than in UA group(P<0.05); The level of interleukin-18 and soluble CD40L were higher in multi- vessel coronary artery lesions group than in single-vessel group(P<0.05).But there is no differentence between double-vessel and triple or more-vessel group(P>0.05);The two markers were positively correlated with the numbers of vessels lesions respectively(r=0.263,P =0.002),(r =0.224,P=0.002). The two markers were positively correlated with the Gensini score respectively (r=0.246,P =0.001),(r =0.365,P=0.001) , The two markers were positively correlated with each other(r=0.352, p=0.002) .Conclusion: As inflammatory cytokines,both IL-18 and sCD40L may take part in the ocurrence and development of ACS, and they have certain predictive value for patients with ACS.The serum levels of IL-18 and sCD40L have positive correlations with each other and the numbers of vessels lesions and the Gensini score respectively ,it indicates that the levels of IL-18 and sCD40L may be used as markers to diagnosis and evaluate the the severity of this disease.
