OBJECTIVE: To establish animal model of laryngocarcinoma,and detect the disseminated tumor cell of peripheral blood and variant P53 in tumour tissue,and judge the correlation of DTC with variant P53 gene and their value of predicting cancer metastasis and prognosis. and the positive of DTC group accepted the treament of Small doses of taxol, and judging its kill DTC, prolong surial and the capability of suppress tumor growth curative effect.METHOD: There are 70 nude mice in the experimental group,and 60 accepted hypodermic injection of laryngocarcinoma cell, 10 in control. By the nested reverse transcription polymerase chain reaction(RT-PCR), we tested CK19mRNA in nude mouse's peripheral blood, as a marker of disseminated cancer cell on the 10 week after tumor-planting. We also tested the expression of P53 gene in tumour tissue by RT-PCR and immunohistology, observe the relationship of DTC and surial,distant organ metastases. The positive group of DTC using small doses of taxol, judge its therapeutic of kill DTC, prolong surial, reduce tumor distant metastases. After the nude dead, we dissect lung,liver and lymph node and use continuous pathological dyeing confirmed whether the transfer. RESULTS: The positive rate of DTC was 66.67%(40/60), but negative in the control group, the difference was significant(P<0.05). In tumor tissue, the positive rate of mutant P53 gene was 60.0%(36/60). The positive rate of mutant P53 protein in the DTC-positive group was 75.0%(30/40); and it was as low as 30.0% in DTC-negative group(6/20).The expression of DTC was correlated with p53(Xˉ2=14.17, P <0.005, r=0.437). The mean one-year the mean survival time of DTC-positive group was 34 weeks vs 43 weeks in negative group(P<0.05) , the check of log-rank is 4.56, the difference was significant (P =0.0327). 6 of the mice whose DTC and p53 are both positive have distant metastasis 42.9%(6/14), 4 Lymph node metastasis,1 liver metastasis, 1 pulmonary metastasis, but in DTC and p53 negative group, liver metastasis was seen in only two case 14.3%(2/14), the difference was significant(P<0.05). Again detection of DTC group was negative, but Saline group was still positive. The saline blank group of survival was 23.1%(3/13), the rate of metastasis was 53.8%(7/13); Small doses of taxol treatment group was 53.8%(7/13), metastasis 14.3%(2/13); DTC negative groups survival was 46.2%(6/13), and metastasis was not to seen, the difference was significant(P<0.05). Kaplan - Meier survival analysis, saline control median survival was 30 weeks, between the group of taxol the check of log-rank was =6.171, the difference was significant (P =0.013). Small doses of taxol can improve the model of survival(P<0.05). The saline blank group and DTC negative group by the check of log-rank was =6.976,p=0.008,revealed that the survival of DTC positive group was significantly lower than negative group(P<0.05); Small doses of taxol and DTC negative group by the check of log-rank was=0.920,p=0.010, the survival of two groups was not significantly differences.CONCLUSIONS: The expression of DTC associated with variant p53 protein could be used as an indicator of tumor metastasis, and it might be an independent risk factor for an unfavourable prognosis; DTC was positive correlation with variant p53 protein. small doses of chemotherapy drug taxol can effectively kill cancer cells, improve prognosis, prolong surial.
