Objective : To explore the clinical value of three-dimensional computed tomography angiography (3D-CTA) in the etiological diagnosis and managements of intracranial aneurysm.Methods: 140 patients with spontaneous spontaneous Subarachnoid Hemorrhage (SAH) admitted from Sep. 2007 to Aug. 2008 were examined with 3D-CTA, then examined with digital subtracted angiography (DSA), among whom, 114 were found suffering from intracranial aneurysm. All the 116 sufferers accepted surgical treatments. 81 cases were executed clipping, and the other 35cases were executed treatment of intervention. Repeated 3D-CTA examinations were performed 1 week after operation. The findings of 3D-CTA examinations were compared with the results of DSA examinations.Results: In the 114 patients, 133 aneurysms were determined by 3D-CTA examination however, DSA detection revealed 132 anurysms. 3 cases of small anterior communicating artery aneurysms, as well as 2 cases of small posterior communicating artery aneurysm conformed by CTA and surgery were found to be negative by DSA examinations. While 4 cases made a definite diagnosis of aneurysm by DSA and operation, were detected to be negative by CTA instead. Two cases of positive CTA examination, in the CTA before and after the inspection done within 15 days of the 1st DSA, DSA inspection results were negative. Two cases of negative CTA examination, DSA examination were positive, the operation proved to be communicating artery aneurysm after the funnel-like expansion.The sensitivity of 3D-CTA diagnosing the intracranial aneurysms is 98.2%, and the specificity is 91.7%. The diagnosis accordance rate among 3D-CTA and DSA and/or surgery is 92.%.Conclusion:The diagnostic outcomes of intracranial aneurysms with 3D-CTA highly accord with 2D-DSA and/or surgery. We should combine the two diagnostic techniques to increase the diagnosis accrracy of intracranial aneurysms and, to reduce misdiagnosis.
