Observation And Analysis On Therapeutic Efficacy Of Ademetionine In Patients With Liver Function Injury

Objective: To observe the therapeutic efficacy and safety of ademe- tionine in patients with liver function injury caused by various etiologies, including improvement of clinical symptoms, liver function parameters and adverse effects, especially its theraputic efficacy in reducing alanine aminotransferase (ALT) and aspartate aminotransferase (AST) level so as to study and evaluate its value in clinical application in improving liver function injury.Methods: Fifty patients with liver function injury caused by various etiologies (mostly chronic viral hepatitis B, liver cirrhosis, alcoholic liver disease, non-alcoholic fatty liver disease, drug-induced liver disease, autoimmune hepatitis) were included in this study. Based on the sequence of enrollment, they were randomly divided into two groups: the therapy group (30 patients) receiving ademetionine 1000mg intravenous bolus qd for 2 weeks and the control group (20 patients) receiving reduced glutathione 1200mg in 5% glucose solution 250ml intravenous guttae qd for 2 weeks besides the same basic liver protective therapy in the two groups. The clinical symptoms including fatigue, loss of appetite, nausea, abdominal distention and discomfort in hypochondriac region, liver function parameters including ALT, AST, albumin (ALB),γ-glutamyl transpeptidase (γ-GT), alkaline phosphatase (ALP), total bilirubin (TBil), direct bilirubin (DBil) and the therapeutic efficacy were observed and compared statistically. Meanwhile, adverse effects of the drug was noticed and recorded.Results: 1. After 2 weeks'treatment , the symptoms of fatigue, loss of appetite and nausea disappeared in both groups. Regarding to the improve- ment of symptom of abdominal distension, the obvious effective rate, the effective rate and the total effective rate in the therapy group were 44%, 44% and 88% respectively, which were 37.5%, 37.5% and 75% in the control group respectively. Regarding to the improvement of the symptom of discomfort in hypochondriac region, the obvious effective rate, the effective rate and the total effective rate in the therapy group were 38.5%, 53.8% and 92.3%, respectively, which were 35.3%, 47.1% and 82.4% in the control group, respectively. The therapeutic efficacy for improving clinical symptoms in the therapy group were not significantly different comparing with the control group (P>0.05).2. The liver function parameters were significantly improved after treatment (P<0.01) in both groups except ALB in the control group. Among the liver function parameters, the serum level of ALT was reduced from 185.6±67.1IU/L to 59.1±35.5IU/L, and that of AST was reduced from 198.9±68.2IU/L to 58.5±40.8IU/L in the therapy group. After 2 weeks'treatment, the serum levels of ALT, AST, ALP, TBil, DBil of the therapy group were all improved significantly when compared with the control group, while ALB andγ-GT were not significantly different (P>0.05).3. The analysis of the therapeutic efficacy for improving liver function parameters indicated that the obvious effective rate 40%, the effective rate 50% and the total effective rate 90% of the therapy group were significantly superior to 20%, 35% and 55% of the control group (P<0.05).4. No obvious adverse effects were not discovered in the course of the treatment in the two groups.Conclusion: 1. Ademetionine is significantly effective in the treatment of liver function injury caused by various etiologies. It improves the symptoms of fatigue, loss of appetite, nausea, abdominal distention, discomfort in hypochondriac region and improves liver function parameters such as ALT, AST, ALB,γ-GT, ALP, TBil, and DBil.2. Ademetionine is safe in clinical application with the high value in improving liver function injury.
