Objective:To make a comparative study on intra-operative manipulation and post-operativepatency between Medical adhesive anastomosis and conventional suturing anastomosis in rabbit external carotid artery. These results provided a experiential evidence for the fundamental investigation to clinical application.Methads:Forty-eight Newzealand white rabbits, which were 2.5Kg, were randomly and equally divided into two groups:control group and experimental group . Each group had 24 Newzealand white rabbits and were devided into four small groups according to the sample times after the operation,Which group 1 (seven day after operation),group 2 (fourteen days after operation),group 3 (twenty-one days after operation),group 4 (one month after operation). Each small group had Three Newzealand white rabbits. Each Newzealand white rabbit was applied as experiment animal, to harvest hemifacial composite-tissue flap based in the common external carotid artery with the branch of theternal mandibular artery and auricularis magna artery, was made an animal model to study the facial transplantation of autografts in rabbits,External carotid artery and vein of every groups'Newzealand white rabbits were cut off and then autotransplantation was performed. We performed the traditional suturing anastomosis method on rabbit common external vein. Experimental group:we performed the end-to-end anastomosis of rabbit common external carotid artery utilizing the medical adhesive. Based on the technical feasibility,the comparable study was made between the Medical adhesive anastomosis method and the traditional suturing anastomosis method. The anastomosingtime and intra-operative blood losswere recorded in operation.and post-oper-ative patency rate intimal hyperplasia degree and the PCNA expression were analyzed and calculated.Results:1,On the whole overview: macroscopic observation of appearance of hemifacial composite-tissue flap, The hemifacial composite-tissue flap on each Newzealand white rabbit was good; And there were not obvious infections and necroses in naked eye. But there were significant differences among different groups from optical observation.The survival area was measured 7 days after the operation .Meanflap survival rate in the experivnental group was (94.87±1.95)%,and control group was(94.88±3.11)%. The differences of the results were not significant (P=0.992>0.01).2,In the experimental group,the anastomosing time was greatly shortened [(8.50±1.93) min vs (22.83±5.04) min], The differences of the results were significant(P=0.00<0.01);the bloodloss was significantly decreased [(2.38±1.41) ml vs (10.00±4.05) ml], The differences of the results were significant(P=0.00<0.01).3,Histopathologic examination: HE dyeing: the intimal hyperplasia of the blood vessel intima in the experimental group was obviously decreased than that of the control group at the 7th,14th,21th,28th day after the operation, The differences of the results were significant(P<0.01).PCNA immunohistological dyeing ; the results suggested that there was rather active PCNA expression on 7,14,21,and 28days after the operation. the PCNA expression in the experimental group was obviously decreased than that of the control group, The differences of the results were significant(P<0.01).Conclusion:The sutureless adhesive anastomosis is easily handled and time-saving and the intra-operative blood loss is decreased.This method decreased the transmural injury by the needle and suture and thus eliminated the proliferative reaction by the foreign bodies such as suture,then the intimal hyperplasia degree decreased and patency rate increased.
