Objective The aim of this report was to introduce the technique of osteotome maxillary sinus floor elevation for implantation by piezoelectric osteotomy. Methods piezoelectric osteotome sinus floor elevation technique was carried out in 33 sites of 20 patients without enough alveolar bone height of maxillary posterior edentulous area. Clinical examination and radiographs were conducted. The residual vertical bone height in the posterior maxilla ranged from 2 to 7 mm.Results 18 patients underwent graft and immediate implant placement, including 5 patients underwent two-stage implant surgery and 13 patients underwent one stage implant surgery ,the other 2 patients were grafted but delayed the implant placement due to the bone height was less than 4 mm. The mean height increase of the implant sites using piezoelectric osteotome sinus floor elevation technique was 5.8mm (range from 2.6 to 14.0 mm). There were no clinical complications of the maxillary sinus area. No implants mobility or lost were noted during a follow-up period of 4 to 19 months. Conclusions Piezoelectric osteotome sinus floor elevation technique greatly simplifies the management process and improves the patient's satisfaction, The technique can be used predictably for implant placement at sites with vertical bony deficiencies in the posterior maxilla.
