PartⅠBenign and malignant pelvic adnexal lesions:Diffusion Weighted Imaging analysis with pathologic correlationObjective:To evaluate the value of DWI and apparent diffusion coefficient (ADC)in distinguish the benign and malignant pelvic adnexal lesions.Material and methods:All images were performed in 1.5T MR scanner.Retrospective analysis the DWI appearance in 117 lesions,which were benign and malignant lesions of pelvic adnexal and confirmed by the pathology.Measured the signal intensity and ADC values of soft tissue when b factors were 50 s/mm2,400 s/mm2 and 800s/mm2,to analyses the rule of signal decrease and to determine the optimal b factor of DWI.The contrast to noise ratio(CNR) and ADC values of soft tissue were compared between benign and malignant lesions.Analysis the features of cystic contents by conventional MRI and DWI.Results:The 800s/mm2 is the optimal b factor in demonstrating pelvic adnexal lesions.The CNR of soft tissue in benign and malignant lesions was 46.25±15.84, 142.25±34.82 respectively,the ADC values was(2.079±0.251) x10-3mm2/s and (1.258±0.332) x10-3mm2/s,CNR of DWI in malignant lesions was significantly higher than that of benign,however the ADC values was lower than benign's,there was statistical significance,the one in the middle of CNR or ADC value was possible of benign or malignant,the feature of DWI and pathology has intimate dependability.The conventional MRI and DWI of cystic contents were varied,the ADC value can predict the component but not distinguish the quality of lesions.Conclusion:The application of conventional MRI and DWI in pelvic adnexal lesions can provide additional information because DWI is a functional imagine.DWI can discriminate benign from malignant by using ADC values of soft tissue and predict the cystic contents,at the same time,it can benefit detecting small lesions and differential diagnosis. PartⅡValue of MRI Diffusion Weighted Imaging in diagnosis of ascitesObjective To summarize the generalization and application of DWI about abdomen and pelvic tumorous,to evaluate the value of DWI in ascites diagnosis. Methods Data including MR presentations,tumor indicators of 188 patients with ascites were analyzed for any difference in sensitivity and specificity between DWI and tumor Indicators.All the patients have performed conventional MR imaging(spin echo T1WI,T2WI and fs-T2WI ),DWI by Siemens 1.5 T MRI scanner,the b value was 800s/mm2.Accuracy of tumor lesions staging determined either by WB-DWI or by non-WB-DWI were also compared for difference,if any.DWI features were summarized.Results The specificity of DWI was significantly higher than that of (92.0%vs33.3%,p<0.05) tumor Indicators.The accuracy of WB-DWI group was significantly higher than that of non-WB-DWI(x2=5.29,p=0.021).There was a statistically significant difference between two groups(P<0.05).Conclusion DWI can discriminate the tumor from non-tumorous lesions and predict malignant ascites. DWI was of excellent differential diagnostic capacity about seroperitoneum (haematoma,empyema )and cystis contents,it can be widely used for patients with ascites.
