Evaluation Of The Effectiveness Of Combined 3D-MRS And Conventional MRI With Body Surface Phased-array Coil In Diagnosis Of Prostate Cancer And Its Kappa Statistics

Objective To evaluate the effect of combined Conventional magnetic resonance imaging(MRI) and 3D-magnetic resonance Spectroscopy (3D-MRS) with body surface phased-array coil in diagnosis of prostate cancer.Materials and Methods 30 men of suspected with prostate cancer,the oldest 86 years old and the youngest 50 years old,average age 70±8 years,PSA＞4ug/Lin 28 cases.30 men underwent conventional T1WI,T2WI,FS-T2WI and 3D-MRS in Siemens Avanto 1.5T MR with body surface phased-array coil,using Spectroscopy software(Siemens Leonardo workstation) for spectral data processing,determining the ratio of(choline+creatine)/citrate((Cho+Cre)/Cit) in the region of interest. Take surgical or system puncture pathology as the gold standard,calculate Kappa statistics and index of sensitivity,specificity,accuracy,positive predictive value, negative predictive value,etc.separately,and compare the diagnostic values of conventional MRI and combined MRI with 3D-MRS diagnosis prostate cancer.Results(1) All of the cases successfully completed the examination,the quality of the spectrum were excellent and the quality of the imaging is good for diacrisis.(2) The Kappa value of conventional MRI diagnosis prostate cancer is 0.429,P=0.018, and has a better consistency with pathological diagnosis.The sensitivity,specificity, accuracy,positive predictive value and the negative predictive value is 83.3%, 58,3%,73.3%,75.0%,and 70.0%respectivel.Kappa value of combined conventional MRI and 3D-MRS is 0.789,P=0.000,and has a good consistency with pathological diagnosis.The sensitivity,specificity,accuracy,positive predictive value and the negative predictive value is 94.4%,83.3%,90.0%,89.5% and 90.9%.Combined use of conventional MRI and3D-MRS can significantly improve The sensitivity,specificity,accuracy,positive predictive value and the negative predictive value than separate use Conventional MRI.Conclusion combined Conventional MRI and 3D-MRS with body surface phased-array coil in Diagnosis of prostate cancer is a good method of magnetic resonance imaging,To some extent,it can be used to replace the endorectal coil,especially suit for those who can not tolerate endorectal coil or there are endorectal coil contraindication.
