Objective:To explore the relevant quantitative indicators reflecting the severity of cervical spondylotic myelopathy(CSM) such as the ratio or difference of cervical spinal cord volume and cervical spinal cancal volume.Searching for quantitative indicators reflecting spinal stenosis degree of CSM.Providing reference reflecting the pathological changes in the degree of spinal stenosis in evaluation of CSM.Methods:1.Inclusion criteria:To select the patients with clinical symptoms consistent with CSM in the orthopedic clinic,ward and the MRI room.Give the patients a JOA score.According with diagnostic criteria of China's second session meeting about CSM,the symptoms of spinal cord compression and T2 weighted MRI showed signal changes consistent with CSM in patients as the patient group.Heaithy volunteers with no chief complaint of CSM are randomly selected as a control group. Do 3D FIESTA sequence of MRI scanning image in both groups.2.To exclude neurological diseases and spinal disc prominent persons suspected broken,amyotrophic lateral sclerosis,Muscular dystrophy,spinal tumors,secondary adhesive arachnoiditis,peripheral neuritis,and other diseases.Collected a total of CSM patients 52 cases,male 23 Cases,female 29 Cases,age 39~80 years old, average 56.1 years old.Selected 38 case healthy individuals as the normal control group.Age 20-30 20 cases,age 50-60 18cases,male 12 cases,female 26 cases.3.MRI measurements:Cervical spinal canal volume and cervical spinal cord volume were measured in MRI workstation.The ratio and difference of cervical spinal cord volume and cervical spinal canal volume were calculated.4.Cervical spinal cord volume,cervical spinal canal volume,the ratio and difference of the above are analysised in Multiple linear correlation with age,height,weight and body mass index.To explore gender differences in the ratio and difference of cervical spinal cord volume and canal volume.To explore the correlation between the JOA score and the ratio and difference of spinal cord volume and canal volume.Results:1.The ratio of cervical spinal cord volume and cervical spinal canal volume was positively correlated with age.2.The ratio of cervical spinal cord volume and cervical spinal canal volume has no correlation with the height.The ratio seems to be affected by body weight.The ratio of cervical spinal cord volume and canal volume in C1-7 and C3-6 has positive correlatio.The ratio of cervical spinal cord volume and canal volume in C3-7 has no correlation in statistics.The ratio of cervical spinal cord volume and canal volume has positively correlation with the body mass index but small correlation coefficients.3.There is no gender difference in the ratio of cervical spinal cord volume and cervical spinal canal volume.4.The ratio of cervical spinal cord volume and canal volume was significant difference in CSM patients group and normal control group;the JOA scores and the ratio of cervical spinal cord volume and canal volume had a negative correlation in CSM group.5.The difference of cervical spinal cord volume and canal volume was negatively correlated with age.The difference is a positive correlation with Height.The difference has no correlation with weight.The difference has negatively correlation with the body mass index but small correlation coefficients.6.The difference of cervical spinal cord volume and canal volume is affected by cervical spinal segment in the correlation of gender.The difference in C1-7 cervical spinal segment is difference in gender,the indicator in C3-7 and C3-6 cervical spinal segment have no difference in gender.7.The difference of cervical spinal cord volume and canal volume was significant difference in CSM patients group and normal control group;the JOA score and the difference had a negative correlation in group of CSM patients.Conclusion:The ratio and difference of cervical spinal cord volume and cervical spinal canal volume can be quantitively reflect the severity of the CSM.The quantitative indicators provide reference for reflecting the pathological changes in the degree of spinal stenosis in clinical evaluation of CSM.
