Objective:Using magnetic resonance imaging(MRI) to quantitatively research normal prostate gland in each age section.Analyze the MRI changes of indicators of the whole prostate and the central zone's diameter volume,T1 and T2 values,and observe the Normal prostate pathological endoscopic.To explore the prostate imaging characteristics and evolution with ageing.And Co-relation analysis the various indicators of the patiens with prostate cancer in a same age group,and Preliminaryly to study diagnosisc value of prostate cancer.Methods:1.MRI study on the normal prostate gland.To examine the 579 healthy volunteers without prostate gland related disease,other urinary system disease with MRI.They were divided into 5 groups according to the age:20~29 year old(119); 30~39 year old(105);40~49 year old(114);50~59 year old(125);above 60 year old(116).To survey the diameter line of prostate gland(Length,Width and Height) and volume,and calculate the volume of the central zone accounts for the proportion of the whole prostate.Set up the reference range of the nonhal prostate diameter,the volume and to analyze its characteristics changes with age growing.2.study the T1,T2 signal intensity in normal prostate:178 normal male adults without any urogenital symptoms included in our study.The volumteers were divided into five groups according to their ages:(same 1).To measure the T1,T2 signal intensity of normal prostate gland and central zone in different age and to evaluate the correlation between signal intensity and age.3.Pathological examination:Selecting the endoscopic biopsy of prostate peripheral zone and central zone of different ages to explore the value of T1 and T2 in the pathological basis of evolution.4.MRI study on the prostate cancer.94 male adults with evidence of prostate cancer included in our study.Ages were from 60~80 years old.The length,width,height and volume of the prostate and central zone were measured in MRI.The volume of certral zone to prostate proportion were calculated.The above datum were compared with the same group of the normal prostate gland..5.Statistical methods Useing(?),s respectively to descripe of discrete trends and the concentration trends of indicators for measuring.using (?)±1.96s to estimate the reference range of Measurement indicators,using independent samples t test or ANOVA comparisons to analyze between the Groups.The same study of the different indicators used paired t test comparison,the linear correlation analysis of pearson was used to analyze the correlation between indicators,useing Logistic regression analysis to Risk assessment. All hypothesis testing standards wereα=0.05 All statistical analysis were used for SPSS15.0.Results:1.The results of diameter line and volume statisticsAverage of upper and lower is 33.95±2.11mm,diameter is 26.40±2.42 mm,Drive is 42.85±3.80 mm,About the scope of the overall volume of the prostate is 11.59~47.01cm3,Its upper and lower tracks,before and after trail,the volume increases with age,There were significant difference in various age groups(P<0.05).Average of upper and lower is 24.87±1.45mm,diameter is 17.49±2.01mm,Drive is 25.46±1.73mm,volume of Prostatic central zone is 2.89~12.46cm3.Its upper and lower tracks,before and after trail,the volume increases with age,volume percentage of the total volume of the prostate accounted for 22%~36%,Its proportion increase with age.Statistical results are statistically significant(P<0.05).2.MRI appearance of the prostate gland and the results of measurement of T1,T2 value and statisticsPeripheral zone's signal was higher than that of central gland on T2WI,the peripheral zone's signal intensity increased gradually with aging while the central gland's signal intensity did not change obviously.Peripheral zone's signal intensity weakened gradually with aging on T1WI,especially after 40 years old.Central gland's signal intensity did not weaken obviously.Comparison of PZ's T1,T2 values under different ages condition showed statistics significance between random two groups(P<0.05).Comparison of CG's T1,T2 value value under different age condition showed no statistics significance between two random groups.(P>0.05).The comparison of peripheral zone's and central gland's T1,T2 value showed statistics significance under the same age condition(P<0.05),except the T1 of 20~29age group(P>0.05).3.the results of Endoscopic biopsy of the prostateIn the puberty the acini and tubular of normal prostate peripheral zone are plently with very few branches.With ageing,Glands were gradualy increased with vacuolization,the acinar were poor.The composition of the central zone of prostate is mainly smooth muscle and matrix-based,Only a small number of acinar increase with ageing,and in the central zone interstitial fiber components increased mainly.Smooth muscle and matrix is no significant changes.4.The results of diameter line and volume statistics with prostate cancerthe percentage of the central zone about Prostate cancer patients(37%~49%) was biger than the same age group of normal prostate(24%~36%),compared with the normal group was significance(P<0.05).The T1,T2 values of foci of prostate cancer were significantly lower.It is Significantly less than the same age with normal peripheral T1,T2 values,There were differences with statistical significance(P<0.05) Reference to the normal group T1,T2 values of the results to risk assessment showed:OR were 0.77,0.73 With the increased of T1, T2 values,Their risk of prostate cancer at reducing.Conclusion:(1) The diameter line,volume of prostate gland and volume of the whole prostate grows gradually following increase of age.(2) The diameter line,volume of central zone and volume of certral zone to prostate proportion grows gradually following increase of age,volume percentage accounted for 22%~36%.(3) Peripheral zone's signal on T1WI weakens gradually following increase of age, central gland's signal does not weaken obviously.(4) Peripheral zone's signal on T2WI increases gradually following increase of age,central gland's signal does not increase obviously.(5) Various diameters Prostate cancer were biger than the normal prostate,volume percentage accounted tbr 37%~49%,T1,T2 values of foci were less than normal prostate.
