Clinical Observation Of Treatment With Intralesional Betamethasone Compound Injection For Active Alopecia Areata

Objective:Alopecia areata is a T cell mediated autoimmune disease.Corticosteroids are the most popular drugs for the treatment of this disease and intralesional corticosteroids is always the first choice abroad.But in our country,few studies have been done evaluating the efficacy of this kind of treatment.Our research is to investigate clinical efficacy and recurrence rate of intralesional betamethasone compound injection treatment of active alopecia areata,and to provide evidence for the better treatment of alopecia areata.Methods:86 active AA patients from the out-patients of the huashan hospital were involved in the study.To evaluate and classify the patients of alopecia areata and separate them for limited AA group,moderate AA group and severe AA group.Once informed consent form signed,participants were given intralesional betamethasone compound injection treatment once three weeks.The efficacy of the treatment was elevated at the third and twelfth week.After the treatment,during a follow-up period of 6 months the recurrence was observed.The data of results were analyzed by SPSS11.0 software.Results:Limited AA group 41 patients(20 male,21 female),the age of the patients ranged from 16 to 54 years(mean,31.1 years);moderate AA group 29 patients(16 male,13 female),the age of the patients ranged from 10 to 53 years(mean,29.5 years);severe AA group 29 patients(7 male,9 female),the age of the patients ranged from 27 to 58 years(mean,33.5 years).The negative conversion rate of the pull test for limited AA group,moderate AA and severe AA group were 80%,76%,12% respectively(after 3 weeks);100%,86%,25%respectively(after 6 weeks).The effective rate for the three groups were 39%,28%,6%respectively(after 6 weeks) were 100%,97%,69%respectively(after 12 weeks).The adverse reaction rate during the treatment were 7%,10%,13%respectively.During a follow-up period of 6 months,the recurrence rate for the three groups were 17%,25%,55%.Conclusion:1.After 3-week treatment,there was no significant difference between the negative conversion rate of the limited AA group and moderate AA group(P>0.05). The negative conversion rate of the limited AA group and moderate AA group was higher than that of the severe AA group(P<0.05).2.After 12-week treatment,there was no significant difference between the total curative rate and total effective rate of the limited AA group and moderate AA group(P>0.05).The total curative rate and total effective rate of the limited AA group and moderate AA group was higher than that of the severe AA group(P<0.05).3.During the treatment,there was no significant difference between the adverse reaction rate of the three groups(P>0.05).4.During a follow-up period of 6 months,there was no significant difference between the recurrence rate of the limited AA group and moderate AA group (P>0.05).The negative conversion rate of the limited AA group and moderate AA group was lower than that of the severe AA group(P<0.05).5.Intralesional betamethasone compound injection treatment of active alopecia areata can be considered more effective for limited AA patients than extensive AA patients.
