Background:Neoadjuvant chemotherapy(NAC) has been widely accepted as the standard management of locally advanced breast cancer in recent years.Though the breast cancer responds relatively well to neoadjuvant chemotherapy,about 20%of patients do not benefit from the different chemotherapy regimens currently in use and are thus subjected to toxic drugs.This often leads to progression of disease and thus the surgeon may lose a window of opportunity to obtain durable locoregional control of disease.Clinically it is very important to identify the reliable predictive markers for the response of neoadjuvant chemotherapy.Purpose:To identify the relationship betweenγ-synuclein,ER,PR,etc with the response of neoadjuvant chemotherapy in breast cancer by observing the response to neoadjuvant chemotherapy and evaluating ER,PR,HER - 2,P53,γ-synuclein,etc before neoadjuvant chemotherapy.Patients and Methods:Between November 2006 and June 2008,91 patients with breast cancer[barring IBC(inflammatory breast cancer)]were treated in the ontology surgery department of Zhejiang University Second Affiliated Hospital,all female,from 27 to 75 years of age.The median age was 51.According to 1997 UICC TNM staging for breast cancer,10 patients in stageⅠ,34 patients in stageⅡA,26 patients in stageⅡB,17 patients in stageⅢA,4 patients in stageⅢC.NAC regimens are FEC,TE/TA and TX,2~6 courses,72(79.1%) patients accepted more than two courses of chemotherapy.Core needle biopsy samples were obtained before the initiation of neoadjuvant chemotherapy.The tissue samples were fixed in 10%neutral formalin and then embedded in paraffin and examined by immunohistochemical staining.Result:After chemotherapy 3 patients showed CR,40 patients showed PR,42 patients showed SD,6 patients showed PD,objective reaction rate(CR+PR) is 47%.In this study ER positive rate is 44%,PR positive rate is 38%,P53 positive rate is 33%,HER -2 overexpressed rate is 34%,SNCG overexpressed rate is 57%.Statistics showed ER and SNCG state correlated to the result of the neoadjuvant chemotherapy.Multivariate logistic regression analysis showedγ-synuclein is the independent predictive factor to neoadjuvant chemotherapy objective reaction rate.Conclusion:Our study showedγ-synuclein has a close relationship to the response of neoadjuvant chemotherapy in breast cancer.
