32 Cases Analysis Of Postransplantation Diabetes Mellitus

Objectives:The development of postransplantation diabetes mellitus(PTDM) is a serious and common complication of kidney transplantation.And PTDM is considered as a key role contributed to graft dysfunction and the survive of patients.This issue studies the incidence of PTDM,and the effect of the Cyclosporine A(CyA) and the Tacrolimus A(Tac)Methods:The patients who were taken the kidney transplantation in kidney disease center,The first Affiliated Hospital,College of Medicine,Zhejiang University from January 2004 to December 2007 are taken into this study.These patients should be followed up for lyear and the fasting blood glucose(FBG) is normal before the transplantation,the number is 504.The definition of PTDM is FBG≥7.0mmol/L twice between 30 days and 365 days after transplantation.The Normal glucose(NG) is defined as FBG＜6.0 mmol/L during the period of following up.We compare the age, days after surgery,dose and concentration of immunosuppressor and kidney function between these two groups.And we divide the PTDM group as CyA subgroup and Tac subgroup,comparing the age,days after surgery,blood glucose and the kidney function between these two subgroupsResults:32 of them are diagnosed as PTDM,the incidence is 6.34%.NG group at the same period is included 472 patients.Compared with normal glucose patients, the PTDM group was older,40.7±11.1 VS 50.8±9.8,P＜0.000001,considered statistically significant.For CyA dose and concentration,the PTDM group was 179.4±50.0mg/d and 238.3±103.1ng/mL,while the NG group was 197.0±55.5 mg/d and 244.1±91.9 ng/mL,For Tac dose and concentration,PTDM was 4.2±3.4 mg/d and 7.8±3.2 ng/mL,at the same time the NG group was 3.9±2.5 mg/d and 7.0±3.6ng/mL.For Creatinine(Cr),is 91.1±31.7 umol/L VS 87.7±22.6 umol/L.All of them are no statistical difference.The Cr is normal in two groups.For CyA group and Tac group.Compare with Tac group,the age,gender,Cr and BUN are ahnost the same, CyA group.The average onset days after transplantation is 183.3±101.4 in CyA group, and the133.5±85.8 in Tac group.The FBG is 8.4±1.2mmol/L in CyA group,while 8.8±1.5mmol/L in Tac group.Both of them are no statistical difference.Conclusion:PTDM is much more taken place in older patients.The PTDM patiedns who take Tac are much earlier than those taking CyA.And for FBG,Tac group is higher than the CyA group,but both of them are no statistical difference.
