Objective:To evaluate the renal function,maily glomerular filtration and analyse the related influencing factors in normoalbuminuric Type 2 diabetic patients.Methods:Two hundred and ninty-two normoalbuminuric(24h urinary excretion, 24h-UAE<30 microg/24h)Type 2 diabetic patients were included in the study. Clinical factors were recorded,blood and renal function parameters were examined,GFR was estimated with simplified equation of MDRD.Multiple linear regression was performed to assess variables independently associated with eGFR.Results:In the 292 normoalbuminuric Type 2 diabetic patients,mean eGFR((?)±S)was 109.6±28.12 ml/(min.l.73m2)-1,64(21.57%)with glomerular hyperfiltration, 161(55.14%)with nomal glomerular filtration and 67(23.29%)with low glomerular filtration.Multiple linear regression analysis disclosed that smoking,the leval of blood urea nitrogen and uric acid along with the insulin resistance index as factors significantly related to eGFR decline,B value were 0.173,-0.103,0.153,-0.299 respectively.Conclusion:A large proportion of the normoalbuminuric Type 2 diabetic patients already had low glomerular filtration.The decresing glomerular filtration rate may be associated with smoking,blood urea nitrogen,uric acid and insulin resistance.It is suggested that we should incorporate assessment of GFR in addition to urinary albumin excretion in order to make early detection of renal disease in Type 2 diabetes.
