ObjectiveA systematic parameter assessment between the two surgical treatments: enucleation and lobectomy.To make an approach of different surgical strategies.And to determine the better choice of operative techniques for resection of the giant hemangiomas of the liver.MethodsRetrospective analysis of 42 patients who underwent the surgical removal of the giant hemangiomas of the liver during June 1999 and January 2009.Lesions of greater than 10 cm is defined"giant hemangiomas of the liver".Regular hepatic lobectomy (Group A) was used for 24 cases and simplex enucleation(Group B) was used for 18 cases.The relative factors between the two different techniques were compared including age,gender,size and location of lesion,operative approach,blood loss, intraoperative transfusion requirement,operative time,pathological examination findings,duration of hospitalization,etc.ResultsAll the 42 cases had successful operation.Those cases with lesions in the left lobe especially in the left lateral lobe were likely usd regular hepatic lobectomy(P <0.05). Enucleation was associated with fewer post-operative complication(P <0.05).Any other parameters such as age,gender,size,blood loss,intraoperative transfusion requirement,operative time,duration of hospitalization in this two groups were similar(P >0.05).ConclusionEnucleation was the first choice in other cases for resection of the cavernous hepatic hemangioma.Those cases with lesions in the left lobe especially in the left lateral lobe were better to use regular hepatic lobectomy.
