Purpose The aim is investigate the fragmented QRS complex in the diagnosis of coronary heart disease (myocardial ischemia), as well as the clinical significance of the joint analysis of the fragmented QRS complex and ischemic ST-T changes in the diagnosis of myocardial ischemia.Method We collected 128 cases of patients hospitalized in the First Hospital of Quanzhou City Heart Medical during 10 September, 2006 to 31 December 2008, and accepted resting ECG surface,coronary angiography, cardiac color Doppler echocardiographyand,Blood biochemical and other related inspection when they hospitalized in our hospital, exclusion out criteria in accordance with the corresponding rule of the possibility of interference caused by a variety of possible cases, the retrospective analysis of 93 cases of the remaining patients. Coronary angiography in the diagnosis of myocardial ischemia as a gold standard, the fragmented QRS complex as a diagnostic indicator of myocardial ischemia at the same time, joint-ischemic ST-T change analysis , Comparison of the two in the diagnosis of myocardial ischemia of clinical significanceResult1. Purely ischemic ST-T changes as a diagnosis of myocardial ischemia in coronary heart disease target, the sensitivity, specificity, positive predictive value, negative predictive value were 56.9%, 50%, 58% and 50% respectively.2. Fragmented QRS complex as a diagnosis of myocardial ischemia in coronary heart disease target, the sensitivity, specificity, positive predictive value, negative predictive value were 49.1%, 64.3%, 60% and 51% respectively.3. Joint analysis of two diagnostic indicators in the diagnosis of myocardial ischemia in coronary heart disease in the sensitivity, specificity, positive predictive value, negative predictive value were71.4%, 60%, 75.5 % and 64.9%ConclusionThe fragmented QRS complex ppear in the surface ECGof in coronary heart disease or suspected coronary heart disease patients can lead the corresponding region of myocardial ischemia,It can be a diagnosis of the current target with the target myocardial ischemia (MI) accepted the diagnosis of ischemic ST-T changes in comparison with a higher specificity,what is more , the study found that the f fragmented QRS complex and ischemic ST-T changes with United, but also will greatly improve the diagnostic sensitivity of myocardial ischemic area and the negative predictive value, its positive predictive value may be significantly higher than pure ischemic ST-T changes. the results suggest that the fragmented QRS complex and ischemic ST-T changes in joint analysis, will help to enhance our work in the clinical diagnosis of myocardial the accuracy of ischemia.
