| Objective: The thesis is to replicate rat model of MODS by two-hit. By observing changes of Exocrine Pancreas function caused by Rats MODS,the thesis is also to investigate the possible effects of somatostatin in Rats MODS.Methods:Forty-five healthy SD rats were randomly divided into three groups: normal control group(group A n=15),MODS model group (group B n=15)and Somatostatin group (group C n=15).The rat models of MODS were made by left legs fracture as'the first hit'and then with Escherichia coli peritoneal injection four hours later as'the second hit'. Each rat of group B received saline(10mg/kg) subcutaneous injection at the following time points :8h,16h,24h,32hand40h. Each rat of group A received saline(10mg/kg) subcutaneous injection at the same time as group B.The Somatostatin group rat was established by subcutaneous injecting octreotide (20ug/kg). The changes of pancreatic, hepatic and pulmonary morphological damage were observed with light microscope in each group.The changes of pancreatic acinar cells, vascular endothelial cells, morphological damage were observed with electron microscope in each group. Try to measure the changes of the serum amylase (AMY),pancreas amylase (PAMY),lypase(LPS),alanine transaminase (ALT),acetic transaminase (ALT ),blood urea nitrogen (BUN), the contents of serum creatinine (CRE);Try to investigate the changes of pancreatic physiological functions in Rats MODS. Octreotide shows protective effects on the multiple organ injury if it is used at the early period of MODS. Regulatory roles of octreotide on expresssion of pancreatic enzyme and inflammatory factor may be considered as one of the therapeutic mechanisms which can be used to treat MODS. RESULTS: TheALT, P-AMY,Cr and BUN were obviously increased in group B.(P<0.05)The chief pathomorphological changes of main organs were non-specific inflammatory response. Compared to control group, pancreatic amylase (PAMY) and lypase(LPS) were remarkedly increased in MODS model,. (P<0.05 ) While this would be changed by Somatostatin; Between the MODS model group and the control group , the difference of the serum amylase was not significant(P>0.05). Compared to MODS model group, pancreatic amylase (PAMY) and lypase(LPS) were remarkedly decreased in Somatostatin group,. (P<0.05 ) Between the MODS model group and the Somatostatin group, the difference of the serum amylase was not significant(P>0.05).Conclusion:The rat model of MODS can be successfully reproduced by two-hit method.The model can be replicated effectively. Moreover,it is very similar in all clinical features to the MODS in human.Octreotide showed its beneficial effects on Pancreas injury in both functional and morphologic alterations during the course of MODS. Regulatory roles of octreotide on expresssion of pancreatic enzyme and inflammatory factor could be considered as one of the major mechanisms under this situation. |
