Objectives:Lichen planus(LP) is a chronic,subacute inflammatory skin disease which occurs predominantly in adults from 30 to 60.The etiopathogenisis and pathogenesis of LP still remains obscure.LP is considered to be correlated with T Lymphocytes by most people,liquefactive degeneration of basal cells main caused by cytotoxic of infiltrative Lymphocytes.So most people consider that LP is related with immunity dysfunction and inflammatory injury.Histopathology of LP shows that hyper Keratosis of epiderm,focal hypergranulosis,acanthosis,typical liquefaction degeneration of basal cells and band-like predominant infiltrating of T cells in the upper dermis.The infiltration of histoleucocyte is one of tissue signature of LP.It is correlated with celluar immunity mediating delayed allergy and chronic inflammation. Chemotatic factor and chemokine receptor play a considerable part in immune-response.MCP-1 is one of chemotatic factorβfamily which come from mononuclear macrophage, endotheliocyte,VSMC,mechanocyte and so on.MCP-1 promotes immigration of histoleucocyte and lymphocyte.It induce cells to release integrin,tissue factor,pre-inflammatory. Chemotaxis of chemotatic factor to WBC is initialized MCP-1 is associated with activation of histoleucocyte,lympho -cyte and basicyte.The chemotaxis of MCP-1 is attribute to CCR2.MCP-1 depend on the expression of CCR2 on endotheliocyte.ICAM-1 is one of adhesion molecule immune globulin.ICAM-1 mediate endotheliocyte adhesion to leukomonocyte,macrophage and promote Inflammatory reaction.ICAM- 1 cohere acceptor LFA- 1 to activate T cell.We propose to observe the expression MCP-1,CCR2 and ICAM-1 in LP lesion by immunohistochemical method in order to disscuse what association these factor with LP and what association among them.Methods:Thirty LP patients,diagnosed by clinic and histopathology,collected from the dermatological derpartment of the Fourth Affiliated Hospital of hebei Medical University from March 2006 to September 2008.The specimens were cut to 4μm serial sections for routine HE Staining,ten normal skin specimens were taken as normal contrals collected from trauma patients.There was no significant difference in sexes and ages between contrals and patients.All data were analyzed using SPASS 13.0 Mann-Whitney U test of rank sum test was used to analyze the result of immuno-histochemistry;Spearman rank correlation was used to analyze the correlation between the expression of MCP-1 CCR2 and ICAM-1.P<0.05 was considered significant.Results:1 Histopathologically results:Thirty specimens showed acanthosis irregularly,liquefaction degeneration of basal cells,band-like inflammatory infiltrate predominantly of T cells.But lesions are differ in degree.Increased angiogenesis and angioectasia was found in dermis superficial lamella of about eighty percent specimens.2 Immunohistochemistry results:MCP-1 expressed positively in intracytoplasm of matpighian cell.MCP-1 expressed almost in full-thickness of LP lesion but in basal layer strongly.MCP-1 expressed weakly positivein normal epidermis, the difference was significant(P<0.05);CCR2 expressed positively in LP lesion,almost in spinous layer and basal layer most strongly in lymphocyte.It is negative in normal epidermis.the difference was significant(P<0.05);ICAM-1 expressed positively in granular layer,prickle cell layer of LP lesion.Few weakly expressed positively in vascular endothelial cell and inflammatory cell It is negative in normal epidermis.the difference was significant(P<0.05).Statistical results of these experiment showed that there was positive associativity between ICAM- 1 and MCP- 1,CCR-2,the values of coefficient correlation and P were respectively:ICAM-1 and MCP-1 r_s=0.720 P=0.000;ICAM-1 and CCR2 r_s=0.754,P=0.000.Conclusions:1 Most of specimens show acanthosis irregularly,liquefaction degeneration of basal cells,band-like inflammatory predominantly infiltrating of T cells and increasing angiectasis in the upper dermis.Increased angiogenesis and angioectasia was found in dermis superficial lamella of about eighty percent specimens.2 This results suggest MCP-1 and receptor CCR2 express strongly in LP.They maybe have chemotaxis and activation to induce monocyte,endotheliocyte express adhesion molecule which make inflammatory cell especially monocyte concentrate to LP lesion.Strongly expression of MCP-1 and receptor CCR2 which migrate lymphocyte to skin from blood vessel and activate lymphocyte.Abnormal chemotaxis migration which induce lymphocyte abnormal biological function so start trail immune response and induce LP.3 Strongly expression of ICAM-1 suggest that it maybe not only participate in vascular endothelial cell and inflammatory cell,but adhesion to malpighian cell which mediate inflammatory reaction of LP.4 Direct correlation between ICAM-1,MCP-1 and CCR2 Suggests they cooperate to promote the lesion of LP.
