Objective: By comparing the internal fixation group with artificial joint replacement group of therapy of the elderly hip fracture. To analyse the treatment effect between the internal fixation and artificial joint replacement therapy.Methods: A retrospective study of treated elderly hip fracture (including the femoral neck and intertrochanteric fracture) of 89 cases in the Third Affiliated Hospital of Guang Xi medical university from March 2003 to March 2008. The patients were divided into two groups. A group is the internal fixation. (including compression screw, dynamic hip screw, Gammar nail or PFN nail.) B group is artificial joint replacement (including the artificial femoral head replacement or total hip replacement). Through the operation records and postoperative follow-up, we compared two groups of the operative time, bleed volume, operative and postoperative blood transfusion volume, X-ray exposure time, postoperative drainage, the time of allow weight-bearing, postoperative complications, postoperative recovery of hip function.Results: By comparing A group with B group in operative time, operative and postoperative blood transfusion, postoperative drainage. There was no significant difference.(p>0.05) but, comparing bleed volume, X-ray exposure time, the time of allow weight-bear after operation, postoperative complications, postoperative recovery of hip function, there was significant difference.(p<0.05)Conclusion: Using the artificial joint replacement to treat the elderly hip fracture, operative X-ray exposure time is short, keep bed time is short after operation, allow early weight-bearing, have low incidence of postoperative complications, early functional recovery of hip is good, more effective than internal fixation in the treatment methods. Long-term effect needs further observation.
