Ramipril Combined With Irbesartan For Treatment Of Heart Failure And Angina Pectoris

Objective To compare the therapeutic effect and safety of Ramipril,Irbesartan and Ramipril combined with Irbesartan for treatment of heart failure and angina pectoris.Methods The 90 patients had got the chronic heart failure and heart function in gradeⅢtoⅣas the description of AHA' standards and the AP grading the Canada cardiac association revised in 1972.the patients with valvular heart disease,hypertrophic cardiomyopathy,perinatal heart disease,active carditis,acute myocardial infarction, systolic blood pressure＜90mmHg or with serious hepatic and renal disease were cancelled. All the selected patients were given the Nitrate-based vasodilating agent,aspirin,digitalis, diuretic andβ-receptor blocker according to their pathogenetic condition,then based on which the group A were given Perindopril,the initial dose was 2.5mg qd,the maximal dose was 10mg/d;the group B were given Irbesartan,and the initial dose was 75mg qd, the maximal dose was 300mg/d.the group C were given the Perindopril or Irbesartan,if the blood pressure was stable,other drug was added a week later,the initial dose and maximal dose of the two drugs were same as the A and B groups,which lasted for 8 weeks.Inquested the patient' pathogenetic condition,medical check-up,cardiac functional grading and recorded the 12 lead electrocardiogram,plasma level of BNP and echocardiogram before and after the treatment.Results The angina pectoris attacks,Nitroglycerin dosage,ST segment move down,brain natriuretic peptide(BNP),the indexes of echocardiography were improved more significantly after treatment in group C than the two other groups(P＜0.05),the frequency of adverse reaction of three groups was similar(P＞0.05).Conclusions Not only does the therapeutic effect of Ramipril combined with Irbesartan for treatment for heart failure and angina pectoris surpass that of Ramipril or Irbesartan alone,but also it is a safe medicine.
