Objective:To investigate the expression of the B7-H4,CA125 and P53 in primary epithelial ovarian cancer(Epithelial Ovarian Cancer,EOC) and their relationship,as well as the relation to clinical or pathological factors of epithelial ovarian cancerMethods:The expression of B7-H4,CA125 and P53 were determined by PV-6000 two-step immunohistochemistry in 70 cases of epithelial ovarian cancer,20 cases of normal ovarian tissue and 20 cases of benign ovarian tumor.To analyze the expression of the B7-H4,CA125 and P53 in primary epithelial ovarian cancer and their relationship through Pearson chi-square test,Fisher's exact test,Kruskal-Wallis H and Spearman correlations.Kaplan-Meier and Cox Regression were used to survival analysis of was epithelial ovarian cancer.Results:The positive rate of B7-H4 in epithelial ovarian cancers was 88.5%(62/70),and it was significantly higher than in benign ovarian epithelial tumors(45%) and normal ovarian tissue (0%).The positive rate of CA125 in epithelial ovarian cancers was77.1%,and it positively correlated with B7-H4.The positive rate of P53 in epithelial ovarian cancer was 68.6%(48/70),significantly lower than B7-H4(P<0.05).The B7-H4's expression in epithelial ovarian cancer was correlate with ovarian canters'differentiation,histological category and whether with lymph node metastasis or not. Multivariate COX regression analysis showed that prognosis of epithelial ovarian cancers were correlated with FIGO(International Federation of Gynecology and Obstetrics) stage,residual tumor size,B7-H4's positive expression,cell differentiation,CA125's positive expression,P53's positive expression and whether lymph node metastasis or not.Single factor Kaplan-Meier analysis showed that the intensity of B7-H4 positive expression is negatively correlated with the average survival time for patients and the cumulative five-year survival rates.Conclusion:B7-H4 overexpress in epithelial ovarian cancer and positively correlates with CA125's expression and higher than P53's expression.The lower cell differentiation of epithelial ovarian cancer, the higher of B7-H4's expression,and the B7-H4's expression in ovarian mucoid adenocarcinoma is stronger than other histological categories epithelial ovarian cancer.B7-H4 is an independent risk factor of total life span of epithelial ovarian cancer patients.The stronger of B7-H4's expression the shorter of average life time,and the lower of cumulative five-year survival rate.B7-H4 should be further investigated as a potential diagnostic and prognostic marker for ovarian cancer,and should be more sensitive and more specific campare to CA125 and P53,meanwhile B7-H4 may offer a fresh idea to the immune adjunctive therapy ofepithelial ovarian cance.
