Objective: Rhinopolypus is one of frequently clinical diseases, which chiefly constitute of dropsically nasal mucous membrane, epidermis is pseudo stratified columnar ciliated epithelium, and exist over proliferation of epidermis cells, under the epidermis is loose connective tissue, and between them is proliferous glandular organ and infiltrative inflammational cells. The nosogenesis of rhinopolypus is indefinite. Recently, some scholars discover that under stimulating with long-time and many kinds of pathological factor, the proliferation and apoptosis disequilibrium and disdifferentiation of epithelium play an impotent role in the course of initial form of rhinopolypus, unceasing accretion of volume, emerging of new glandular organ and inflammatory reaction of mucous membrane. XIAP (X-linked inhibitor of apoptosis) is the most effective in IAP (inhibitor of apoptosis) family. It can inhibit the caspase (The caspase family as a key molecule group can regulate the apoptosis. Caspase-3 has the closest relation with apoptosis, and it participate in the apoptosis which induced by many factors. ) directly and block the formation and development of apoptosis. The purpose of this study is examine the expression of XIAP in the rhinopolyp, trying to discuss the relationship of XIAP and eosinophile granulocyte, investigate the possible role of XIAP in rhinopolyp and further explores the pathogenesis of rhinopolyp.Method: Specimens of polyps from twenty-five rhinosinusitis patients and inferior turbinates from ten normal patients receiving septoplasty and partial inferior turbinectomy were collected, prepared and stained by hematoxylin-eosin staining and immunohistochemical method using polyclonal antibodies of XIAP and staining method. The expression of XIAP in epithelia, glandular cells as well as inflammatory cells and eosinophile granulocyte (EOS) were copmpared and analyzed.Results: 1. The expression of XIAP in epithelium cells, glandular epithelium cells and inflammation cells of rhinopolypus was less than that from normal controls, with the expression of XIAP in inflammation cells and glandular cells being more than that of epithelium cells (statistically not significant). Compared with that from normal inferior turbinates, the expression of XIAP protein in rhinopolypus decreased obviously (Z=-2.411,P<0.05). 2. The infiltration of EOS in rhinopolypus was much richer than that in normal controls. (Z=-2.877,P<0.01). 3. The correlated analysis of XIAP expression and EOS infiltration in different sites in rhinopolypus shown that the expression of XIAP protein was inversely correlated with EOS infiltration (rp=-0.569,P<0.01).Conclusion: 1. The expression of XIAP in epithelium cell, glandular epithelium cell and inflammation cell of normal inferior turbinates can show that the system of XIAP maybe participate the cell renewing and oneself steadying of normal nasal mucous membrane. 2. There was less expression of XIAP in epithelium cell, glandular epithelium cell and inflammation cell of rhinopolyp, the expression of XIAP in inflammation cell and glandular epithelium cell more than epithelium cell, but not statistically significant. It illustrates that the apoptosis ability of rhinopolyp go up, especially in inflammation cell. 3. There was more expression of eosinophile granulocyte in rhinopolyp. Comparing with normal inferior turbinates, the expression of eosinophile granulocyte significantly increased that in rhinopolyp, implies that there is exist obvious proliferation in rhinopolyp. 4. The correlated analysis in rhinopolyp was shown: the expression of XIAP protein was inversely correlated with eosinophile granulocyte. We inferred that the expression of XIAP protein obviously decreased in rhinopolyp can relief the inhibition of Caspase-3, thus, can accelerate the apoptosis of rhinopolyp, but in the same time exists the proliferation of eosinophile granulocyte, thereby, it breaks the balance of normal proliferation and apoptosis. The lower expression of XIAP and rich infiltration of EOS in rhinopolypus shown that the level of apoptosis and proliferation of inflammation cells are both up-regulated, indicating that both apoptosis of cells and EOS play an important role in the pathogenesis of rhinopolypus.
