The Clinical Study Of The Therapy Of Jianpihuoxue For E Antigen-negative Chronic Hepatitis B

Objective:Viral hepatitis,especially hepatitis B and its complications,is a kind of disease threatening severely people' s health.Respecting the disease lacking method having special good effect hitherto,the feature in the whole concept and the determination of treatment based in pathogenesis obtained through differentiation of symptoms and signs of Traditional Chinese Medicine,and the curative mode concerning the whole and the individual in the disease prevention and cure,show powerful ascendancy in the prevention and cure of HVB.The researches and the practical consequences in several years indicate that,the method to prevent and cure hepatopathy such as VH and hepatic cirrhosis using Chinese medicine is hardly to be replaced by the other methods, so it is significance in ensuring people' health that we strengthen the research treating this disease using Chinese medicine,developing the characteristic of TCM,approaching the rule to cure this disease in the way of determination of treatment based in pathogenesis obtained through differentiation of symptoms and signs,opening up an efficacious curative way. In this topic,we compose jianpihuoxue prescription in the principle of strengthening spleen and promoting blood circulation,and contrast the therapeutic efficacy in the way of strengthening spleen and promoting blood circulation with that of adefovir dipivoxil in chronic HVB which Ag E is negative,approach the curative method much more effective and economical in treating chronic HVB which Ag E is negative.Methods:We chose HBV patients in accordance with inclusion criterion,who visited the out-patient of Hepatitis Branch intestinal of the first affiliated hospital of Guang Zhou University of TCM between 1/2008 and 10/2008, and grouped them into 3 groups randomly(jianpihuoxue description group 20 cases,adefovir dipivoxil group 19 cases and therapeutic alliance group 21 cases).We separately detected ALT,AST and HBV-DNA of each group before the treatment and 3,6 month after the treatment,observing the changes of each breadboard index before and after the treatment.Results:1.The total effective rate of group jianpihuoxue is 35%,group adefovir dipivoxil 78.94%and group therapeutic alliance 80.96%.There is significant difference between group jianpihuoxue and the other group in the curative effect;there is not statistically significant between group adefovir dipivoxil and group therapeutic alliance,and the total effective rate of group therapeutic alliance is the biggest,what shows that the curative effect of group therapeutic alliance is the best.2.The ALT and AST of each group reduced after the treatment distinctly(P＜0.05).The contrast between group jianpihuoxue and the other group at the aspect of ALT and AST is significant difference,thus no statistically significant between group adefovir dipivoxil and group therapeutic alliance(P＜0.05).It shows that the effect to cut down the enzyme of group jianpihuoxue is more inferior than that of group adefovir dipivoxil, but it can raise the effect to cut down the enzyme if we use the group jianpihuoxue and group adefovir dipivoxil as an alliance.3.The dynamic observing of ALT and AST before the treatment and 3,6 month shows that all of them fall-off along with course of treatment lengthening. The effect treating for 6 months surpass that treating for 3 months.4.HBV-DNA quantitation of all groups detected at the 6th month after the treatment reduce,compared with that detected before the treatment,what shows that both the jianpihuoxue prescription and adefovir dipivoxil can restrain the virus from copying;in the aspect of HBC-DNA reducing,adefovir dipivoxil surpasses jianpihuoxue prescription(P＜0.05).The contrast in virus reducing between the way using jianpihuoxue prescription as an alliance and the one using adefovir dipivoxil alone is not statistically significant(P＜0.05).5.There are some HBV-DNA darkening after the treatment in all groups.The contrast of HBV-DNA diverting between jianpihuoxue prescription and the other groups is significant difference(P＜0.01),while no statistically significant in the contrast of HBV-DNA darkening between the group adefovir dipivoxil and group therapeutic alliance(P＞0.05).In the aspect of darkening,the effect of group jianpihuoxue prescription is obviously inferior group adefovir dipivoxil.6.There is not statistically significant in the contrast of improving such as lassitude,chest distress,belching,chest and hypochondrium,loose stool,anorexia,among the three groups after the treatment(P＞0.05).In the aspect of symptom such as bitter taste of mouth,abdominal distention,restlessness,tantrum,nausea and vomiting,the contrast between group adefovir dipivoxil and the other groups is statistical significance.It shows that in the symptom improving such as lassitude,chest distress,belching,chest and hypochondrium,loose stool,anorexia,the effect using jianpihuoxue prescription alone and using adefovir dipivoxil as alliance is better than that using adefovir dipivoxil.Conclusion:Both jianpihuoxue prescription and adefovir dipivoxil have the effect of reducing enzyme,protecting liver,improving Chinese symptom,stopping virus from copying.But in the aspect of invigorate the spleen and promoting blood flow,jianpihuoxue prescription is better than that adefovir dipivoxil,while the total effective rate of alliance in the aspect of the curative effect.Above all,the effect treating HBV with antigen E negative is obvious good allying jianpihuoxue prescription with adefovir dipivoxil.
