Objective:To evaluate the myocardial protective effects of HTK on the operation of aortic dissection,through comparison with 1:4 cold blood cardioplegia and HTK cardioplegia.Methods:Nineteen patients with aortic dissection were randomized to receive 4:1 cold blood(group A,n=7)or HTK(group B,n=12)cardioplegic solution during the operation.Get the blood sample and measure the serum levels of cardiac troponin(cTn I),creatine kinase MB(CK-MB) immediately before operation and after the heart beating 1 h,2h,6h,12,24h,48h.The following clinical parameters were recorded, such as the auto-beating ratio,the inotropic support requirement,the hours and length-of-stay in the intensive care unit,etc.Myocardial ultrastructures were observed.Results:The concentration of cTnI and CK-MB were all increased significantly after the operations,and serum levels of cardiac cTnT and CK-MB in group B were lower than that in groug A(P<0.05).The dose of inotropic drugs,the hours of mechanical ventilation and length-of-stay in the intensive care unit were lower in group B(P<0. 05).Electron microscopy showed that the mitochondria in group B were better than that in group A(P<0.05).Conclusions:HTK cardioplegic solution is value for myocardial protection during the operation of aortic dissection.
