Study On The Relationship Between Serum OPG/RANKL And Arterial Calcification In Chronic Renal Failure Patients On Heamodialysis

ObjectiveTo investigate the variation of serum OPG/RANKL levels in chronic renal failure(CRF) patients on heamodialysis,and to elucidate the correlation of serum OPG/RANKL levels and the extent of arterial calcification,arterial elasticity.MethodsWe selected 108 CRF patients on heamodialysis and 32 normal subjects for this study.Serum OPG/RANKL levels were determined by ELISA.Two dimensional ultrasound of Carotid,abdominal aorta,iliofemoral axis and legs was performed in all subjects.Intima-media thickness(IMT) of carotid was measured at the same time.Arterial calcification at each arterial site was quantified qualitatively as absent(0) or present(1).The final overall score was obtained by the addition of calcifications from all studied zones.The final score ranged from 0 (absence of calcium deposit) to 4(calcifications present in all arterial segments examined).E-Tracking technology was performed to measure the stiffness parameter(β),elasticity modulus(Ep) and arterial compliance(AC) of common carotid.All patients were divided into 5 groups including group 0(no calcification,26 patients),group 1(calcification in one artery,15 patients),group 2 group 1(calcification in two arteries,10 patients ),group 3(calcification in three arteries,11 patients),group 4(calcification in four arteries,46 patients).The differences of serum OPG,RNAKL among different groups were analyzed,and the relationship between OPG,RANKL and the extent of artery calcification and elasticity was also analyzed.Results1.Clinical characteristice of the study populations Systolic blood pressure,diastolic blood pressure,glucose,creatinine,blood urea nitrogen,serum potassium and serum phosphorous were significantly increased,and total cholesterol,calcium hemoglobin were significantly decreased in patients on heamodialysis when compared with controls.Other parameters showed no significant difference between two groups.2.Comparison of arterial calcification and elasticity between CRF patients and controlsPercentage of calcification score 4 in patients on heamodialysis was significantly increased when compared with controls(43%vs.16%,P<0.05).βand Ep were significantly increased in patients on heamodialysis when compared with controls(P<0.01).AC showed no difference between two groups.3.Seri,OPG/RANKL levels between tow groupsSerum OPG levels was significantly increased,and serum RANKL levels was significantly decreased in CRF patients when compared with controls (P<0.05-0.01),and showed no significant difference between sexes.4.Comparison of arterial elastivity among six groupsβwas significantly increased in group 4when compared with other groups. (P<0.05-0.001);βwas significantly increased in group 3 when compared with group 0 and 1(P<0.05-0.01 )。Ep was significantly increased in group 3 and 4 when compared with controls,group 0 and 1(P<0.05-0.001);Ep was significantly increased in group 4 when compared with group 2(P<0.01).AC was significantly increased in group 0,and significantly decreased in group 4,when compared with controls.5.Comparison of serum OPG and RANKL levels among different groupsSerum OPG levels was significantly increased in group 0,1,2,3 and 4 when compared with controls(P<0.01-0.001);Serum OPG was significantly increased in group 2,3and 4 when compared with group 0 (P<0.05-0.001);Serum OPG was increased in group 3and 4 when compared with group 1 and 2(P<0.05-0.001).Serum RANKL levels was significantly decreased in group 4 when compared with controls,group 0,1 and 2(P<0.05-0.001).6.Analysis of correlations among serum OPG and Rankl levels, clinical characteristics in CRF patients on heamodialysisSerum OPG levels was positively correlated with the duration of dialysis and the age,and negatively correlated with serum RANKL levels(P<0.05-0.001)7.Analysis of corre;ation among serum OPG and Rankl levers, ultrasonagranbic datas of carotid,and elastivity of carotid in patients on heamodialysisSerum OPG was positively correlated with the extent of arterial calcification, IMT of carotid,Ep,andβ,and negatively correlated with AC(P<0.05-0.001).Serum RANKL levels was positively correlated with AC,and negatively correlated with Ep(P<0.05-0.01).Conclusion1.Serum OPG was increased,and RANKL was decreased in CRF partients on heamodialysis.Serum OPG levels was correlated with serum RANKL negatively..2.Serum OPG levels were positively correlated with age,the duration on dialysis,and IMT of carotid.3.Serum OPG levels increased with the increased extent ofarterial calcification,and serum OPG may reflected the extent of arterial calcification.4.Serum OPG correlated positively with(β,Ep,and negatively with AC.
