| Objective Cancer prevalence and trends are a measure of disease burden in the general population.In order to provide a scientific basis data for preventting cancers we analysis the medical cost and the trends of common cancer cases in female from 1996 to 2006 in a city and forecast the trends in 2010.Method It is the survey that cases of cervical cancer,breast cancer,ovarian cancer and endometrial cancer what were Diagnosised that came from a general hospital and a specialist hospital in a city,extracted medical records and acquired interrelated messages,adjusted hospitalization costs with annual interest rate of every years,and evaluated economic burden of disease and cost-effect analysis.Forecast the tendency of change of the four kinds of cancer by fitting exponential curve and Linear curve from 2007 to 2011.Result(1) extracted 5564 valid medical records,including 1618 cases of cervical cancer(29.08%),3107 cases of breast cancer(55.84%),617 cases of ovarian cancer(11.09%),222 cases of endometrial carcinoma(3.99%).The average age of patients of cervical cancer,breast cancer,ovarian cancer and endometrial cancer respectively was 48.87±10.19 yeas old,48.60±10.72 years old,48.35±12.62 years old and 54.25±10.02 years old.There was significant difference between occupational distribution of the four kinds of cancer.Most patients of cervical cancer were farmers,the proportion was 62.13%;most of breast cancer were Workers,the proportion was 29.43%;most of ovarian cancer and endometrial cancer were unemployed,the proportion respectively was 24.30%and 29.29%;Occupational distribution of cervical cancer,breast cancer and ovarian cancer in different years were no significant difference(p＞0.05),but that of endometrial cancer were significant difference(p＜0.05).(2) Therapeutic modalities of the four kinds of cancers all showed remarkable significance(p＜0.05).The proportion of conservative treatment and operation of cervical cancer respectively was 39.19%and 60.81%,breast cancer respectively was 47.22%and 52.78%,ovarian cancer respectively was 66.94%and 33.06%, endometrial cancer respectively was 33.19%and 60.81%.(3) Treatment outcomes were statistically significant differences between of the therapeutic modalities of the four kinds of cancers.effective rate of operation of cervical cancer, breast cancer,ovarian cancer and endometrial cancer respectively was 97.47%,95.96%,96.57%and 99.26%,that of conservative treatment respectively was91.61%,88.76%,85.71%and 89.66%.Treatment outcomes had no statistics difference among different years of cervical cancer,ovarian cancer and endometrial cancer,but that of breast cancer had statistics significant difference,cure rate of breast cancer ranked the top three respectively was 1997,1996 and 1998,the cure rate respectively was 77.40%,77.16%and 71.83%,and there were a linear trend between treatment outcomes and years(x~2=46.16,p=0.00).(4) hospitalization expenses per patient of cervical cancer,breast cancer,ovarian cancer and endometrial cancer all had statistics significant difference among different years,the value respectively was 6329.98±2.47 yuan,9684.22±2.19 yuan,4933.18±2.48 yuan and 7207.37±2.08 yuan, and hospitalization expenses per patient of the four kinds of cancer were all increased annually,hospitalization expenses per patient of breast cancer was higher than that of other types of cancer.Every individual hospital costs in different were all increased annually.The average annual growth rate of individual hospital costs ranked the top three respectively was surgical fees,inspection fees and other costs,the value respectively was 29.01%,15.17%and 10.56%.hospitalization expenses per patient had statistics significant difference among different Occupational people.The hospitalization expenses per patient for cardres were the highest,but the ones for peasents were the lowest,the value respectively was 9158.77±2.47 yuan and 5408.87±2.25 yuan.Both hospitalized patients and hospitalization expenses per patient had statistics significant difference among different pay way for medical care in different years.Patients who have to bear the full medical expenses of there own were most,there were about 3444 ones,patient who pay for medical costs with other ways were least,there were only 8 ones.The hospitalization expenses per patient for patients who pay for medical costs with business insurance were the highest,but the ones for patients who have to bear the full medical expenses of there own were the lowest,the value respectively was 18169.74±2.22 yuan and 7576.63±2.33 yuan.(5)cost-effectiveness analysis:marginal cost effectiveness(△C/△E) of different ways of treating cervical cancer,breast cancer,ovarian cancer and endometrial cancer respectively was 42.54 yuan,8.50 yuan,6.39 yuan and 26.02 yuan,that of different hospital treating the four kinds of cancers respectively was -24.06 yuan,-9.37 yuan,1.43 yuan and 3.87 yuan.(6) The average annual growth rate of patients and hospitalization expenses per patient of common female cancers will respectively be 6.79%and 4.85%in nexe five years.Conclusion(1)Hospitalized patients of cervical cancer,breast cancer,ovarian cancer and endometrial cancer all had increasing trend,the average age of the four kind of cancers were also lower than home and abroad,and all had younger trend.Most of cervical cancer were peasents,most of breast cancer,ovarian cancer and endometrial cancer were all urban dwellers.(2)Early diagnosis rate of cervical cancer,breast cancer and ovarian cancer all had no obvious increase,curative effects also didn't make any breakthrough in resent years.But Early diagnosis rate of endometrial cancer was higher,and its curative effect was also better.cure rate of Common female cancers cure rate increase still depends on its rise in the level of early diagnosis.(3) Direct medical cost of common female cancers showed obviously rising trend,and its indirect cost also increased.patients and hospitalization expenses per patient of common female cancers will increase respectively with the average annual growth rate of 6.79%and 4.85%in nexe five years.It undoubtedly brings tremendous burden of disease and economic pressure to family and society.therefore,we recommends that the government take corresponding measures,increase the intensity of investment in cancer prevention,achieve early detection and treatment,to reduce medical costs and relieve the economic burden of family and society. |
